摘　　要：目的　观测老年糖尿病心率变异 (HRV)的变化及对诊断心脏自主神经病变的意义。　 方法　老年糖尿病组 3 6例及正常老年对照组 40例进行 2 4h动态心电图监测。HRV时域分析指标为 :2 4h平均正常R R间期标准差 (SDNN) ,连续 5min正常R R间期标准差 (SDANNindex) ,连续 5min正常R R间期标准差的均值(SDNNindex) ,相邻正常R R间期差值均方根 (rMSSD)。　 结果　糖尿病组SDNN ( 83 2 8± 3 7 5 9)ms ,SDANNin dex( 75 1± 3 6 49)ms ,SDNNindex( 3 1 0 3± 16 0 9)ms ,rMSSD( 2 1 13± 2 0 5 9)ms均低于正常对照组 (P <0 0 5 ) ,其中rMSSD病变发生率最高 ( 5 0 % )。　 结论 　HRV可作为诊断老年糖尿病早期心脏自主神经障碍的一种方法 。Objective To observe the alterations of heart rate variability (HRV) in the aged patients with diabetes and the diagnostic significance of autonomic neuropathy Methods A group of 36 aged patients with diabetes and a controlled group of 40 healthy aged persons were monitored by continuous 24 hour DCG. The indicators of HRV time domain analysis are: SDNN, SDANNindex,SDNNindex and rMSSD. Results The SDNN(83 28±37 59)ms,SDANN index(75 11±36 49)ms, SDNN index (31 03±16 09)ms and rMSSD(21 13±20 59) for the group of diabetic patients, and all lower than the healthy group ,in DM group,rMSSD the incidence of abnormal were 50%. Conclusions HRV can be applied to diagnose the early stage autonomic neural dysfunction for the aged patients with diabetes. and is practical for prevent serious complication in time.
