小剂量舒芬太尼复合丙泊酚用于无痛纤维结肠镜检查术的效果

摘　　要：目的:观察小剂量舒芬太尼复合丙泊酚用于无痛纤维结肠镜检查术的麻醉效果及安全性。方法:80例ASAI或Ⅱ级行纤维结肠镜检查术患者,随机分为小剂量舒芬太尼复合丙泊酚组(A组)和丙泊酚组(B组)。每组40例,A组5 min内缓慢静脉滴注小剂量舒芬太尼0.1μg/kg,再静脉滴注丙泊酚1 mg/kg;B组单独静脉滴注丙泊酚2mg/kg,必要时追加丙泊酚。对两组患者的麻醉镇痛效果进行评级;观察两组患者注药前(T0)、注药后2 min(T1)、过脾曲时(T2)、过肝曲时(T3)及苏醒时(T4)HR、SBP、SpO2的变化;以及起效时间、苏醒时间、丙泊酚用量、离院时间和麻醉后不良反应发生情况。结果:A组镇痛效果优于B组(P<0.05)。T1时两组HR慢于、SBP及SpO2均明显低于T0时(P<0.05)。B组起效、苏醒和离院时间长于A组(P<0.05),B组丙泊酚用量明显高于A组。两组均未出现严重的低血压、心动过缓、呼吸抑制,常规面罩给氧,SpO2均为99%以上。结论:小剂量舒芬太尼复合丙泊酚可以安全用于无痛结肠镜检查且麻醉效果满意。Objective Observation of small dose sufentanil and propofol used in painless colonoscopy anesthetic effect and safety of operation. Methods 80 patients with ASAI or IIgrade fiberoptic colonoseopy patients, rando rely divided into small dose sufentanil and propofol group （ group A）and propofol group（B group） .40 patients in each group,A group 5 minutes slow intravenous injection of small dose sufentanil 0.1 ug/kg, and intravenous propofol 1 mg/kg;B group separate intravenous propofol 2 mg/kg, if necessary, additional propofol. Two groups of patients with anal gesic efficacy rating;observation of two groups of patients before injection（T0 ）, 2 minutes after injection （T~）, splenic flexure （T2 ）, when the liver piece（T3 ） and wake up（＂1＂4 ） HR, SBP, SpO2 changes; as well as the onset time, recovery time, the dosage of propofol anesthesia, hospital stay time and after the occurrence of adverse reactions. Results In A group, the anal gesic effect was better than group B （P 〈 0.05 ）. Two Tl HR, SBP and slow in group SpO2 were significantly lower than in To （ P 〈 0.05）. Group B onset, recovery and hospital stay time longer than group A（ P 〈 0.05 ）, the dosage of propofol in group B was higher than that in group A. The two groups were not severe hypotension, bradycardia, heartbeat respiratory inhibition,conventional face masks to oxygen, SpO2 are more than 99% .Conclusion Small dose sufentanil and propofol can be safely used in painless colonoseopy and anesthetic effect satisfaction.
