肾结石合并非特导性尿路感染的临床研究

摘　　要：目的探讨肾结石合并非特异性尿路感染患者的细菌学特点，了解与细菌感染的临床相关因素及细菌感染对微创经皮肾镜手术后发热、出血的影响。方法回顾性分析1132例肾结石患者中段尿培养结果，分析尿培养结果与性别、结石种类、肾积水、肾功能的关系，以及与微创经皮肾镜手术后发热、出血的关系。结果1132例肾结石患者中尿培养阳性396例，占34．99％，其中革兰阴性菌294例，占74．24％，革兰阳性菌102例，占25．76％；尿培养结果与性别、结石种类、肾积水、肾功能等明显相关，按尿培养结果分尿培养阳性组和阴性组，尿培养阳性组术后发热及出血的比率较阴性组明显升高，差异有统计学意义（P〈0．01）。结论肾结石合并非特异性感染菌种分布广，细菌感染与性别、结石种类、肾积水、肾功能等临床因素有关，合并细菌感染的肾结石患者术后发热的并发症较非感染者明显升高。Objective To investigate the bacterial category, and understand the clinical related factors of bacterial infection, the effect of bacterial infection on postoperative fever and homorrage of minimal invasive percutaneons nephrolithotomy. Methods A retrospective analysis was performed in 1132 patients with renal calculi. The relationships between urine culture results and gender, stone types, hydronephrosis and renal function were analyzed. And the relationships between urine culture results and postoperative fever and bleeding of minimally invasive percutaneous nephrolithotomy was analyzed. Resuits Urine culture was positive in 369 cases（34. 99% ） among the 1132 cases of renal calculi, and Gram-negative bacteria contributed to74. 24% （294/396） of the total bacteria and Gram-positive bacteria contributed 25.76% （102/396）. There were positive correlations of urine culture results with gender, stone types, hydronephrosis, renal function. According to the results of urine culture, there were significant differences in postoperative fever and bleeding rates between positive culture group and negative cul- ture group （P 〈 0. 01 ）. Conclusions The bacterial category of renal calculi complicated with nonspecific infection is broad, and the bacterial infection is correlated with gender, stone types, hydronephrosis and renal function. The complication of postoperative fever is significantly elevated in culture positive group.
