摘  要:目的探讨血浆热休克蛋白70(HSP70)在Graves病(GD)发病机制中的作用。方法选取重庆医科大学附属第一医院内分泌门诊2010年3—7月GD初发患者25例(病例组)和健康体检者25例(对照组),用酶联免疫吸附试验(ELISA)测定两组受检者血浆肿瘤坏死因子α(TNF-α)、HSP70水平,采用比色法测定血浆丙二醛(MDA)水平,同时采用反转录聚合酶链反应(RT-PCR)测定外周血单个核细胞Toll样受体4(TLR4)及核因子κB(NF-κB)的mRNA表达。结果病例组患者血浆HSP70、TNF-α、MDA水平均显著高于对照组(P<0.05);血浆HSP70水平与TNF-α、MDA、游离三碘甲腺原氨酸(FT3)、游离甲状腺素(FT4)呈正相关(r值分别为0.483、0.412、0.327、0.445,P<0.05)呈正相关,而与促甲状腺素(sTSH)呈负相关(r=-0.315,P=0.045)。GD患者外周血单核细胞TLR4及NF-κB mRNA表达量均显著高于对照组,差异有统计学意义(P<0.01)。结论 GD患者血浆HSP70水平升高,与健康体检者相比,血浆HSP70可能作为一种活性细胞因子参与GD的免疫炎症反应。Objective To investigate the role of heat shock protein 70 (HSP70) in the pathogenesis of Graves disease (GD) . Methods 25 new onset GD patients (ease group) and 25 healthy subjects (control group) admitted to the Department of Endocrinology of the First Affiliated Hospital of Chongqing Medical University from March 2010 to July 2010 were involved into the study. Enzyme linked immunosorbent assay (ELISA) was used to determine the levels of tumor necrosis factor α (TNF-α) and heat shock protion70 (HSPT0) . Colourimetry was used to determine the levels of malondialdehyde (MDA) and RT - PCR was used to determine the levels of toll like receptor 4 ( TLR4 ) and the mRNA expression of nuclear factor - KB ( NF - KB ). Results The levels of serum HSP70, TNF -α and MDA were significantly higher in ease group than in control group ( P 〈 0.05 ) . The level of serum HSP70 was positively correlated with TNF-α, MDA, b-T3 and FT4 (r = 0. 483, 0. 412, 0. 327 and 0. 445 respectively, P 〈0. 05), but was negatively correlated with sTSH (r = -0. 315, P =0. 045) . The levels of TLR4 and mRNA expression of NF - KB in peripheral blood mononuelear cell were significantly higher in GD patients than in control group ( P 〈 0.01 ) . Conchmion Serum HSP70 Level increased in case group, compared with control group, HSP70 may take part in the GD pathogenesis as a kind of immunologic cellular activity factor and may play an important role in the immunopathogenesis of GD.
