舒洛地特对糖尿病下肢血管病变的疗效研究

摘　　要：目的观察舒洛地特治疗糖尿病下肢血管病变的效果。方法选取30例2型糖尿病合并下肢血管病变患者,给予舒洛地特治疗3个月,前15 d给予舒洛地特注射液120 mg,1次/d,肌肉注射或皮下注射;后2个半月给予舒洛地特胶囊50 mg/次,2次/d,口服。采用自身前后对照的方法,比较用药前后患者下肢血管病变和神经病变的症状、无痛行走距离、能耐受疼痛的最大行走距离、踝肱指数(ABI)、足背动脉血流动力学及代谢指标的变化。结果舒洛地特治疗后患者的最大行走距离、无痛行走距离均较治疗前显著增加(P<0.01);足背动脉的平均血流速度、搏动指数、阻力指数及ABI显著改善;总胆固醇、三酰甘油、低密度脂蛋白、纤维蛋白原、C反应蛋白水平较治疗前显著降低(P<0.05)。结论舒洛地特治疗糖尿病下肢血管病变,可通过降低周围动脉阻力指数,增加搏动指数,改善周围血管血流量,有效增加患者最大行走距离及无痛行走距离,且无严重不良反应。Objective To study the effect of sulodexide on peripheral artery disease （PAD） in patients with type 2 di- abetes. Methods 30 diabetes patients with PAD were involved into the study. The patients were treated with sulodexide for three months, during which the patients were given 120 mg sulodexide injection once a day through intramuscular injection or subcuta- neous injection in the first 15 days and given sulodexide capsule per os twice a day with each time 50 mg. PAD, neuropathy, pain - free walking distance, maximal walking distance, ankle brachial index （ ABI）, ultrasonic angiography image of the peripheral vessels and metabolic indexes were compared before and after treatment. Results Both pain - free walking distance and maximal walking distance were significantly increased after the treatment of sulodexide （ P 〈0. 01 ） . Mean blood flow velocity in dorsum of foot, pulsatile index, resistance index and ABI were significantly improved. Total cholesterol, triglyceride, LDL - C, fibrinogen and CRP were significantly decreased after treatment compared with those before the treatment （P 〈 0 05） . Conclusion Sulo- dexide can decrease vascular resistance index, increase pulsatile index and velocity of bloodstream, and it can also increase pain - free and maximal walking distance without severe adverse effect in the treatment of diabetes with PAD.
