PPH联合外痔剥离术治疗急性嵌顿环状混合痔34例疗效观察

摘　　要：目的:比较痔上黏膜环切钉合术联合外痔剥离术与传统外切内扎术治疗急性嵌顿环状混合痔的临床治疗效果。方法:将本院收治的68例急性嵌顿环状混合痔患者随机分为A组与B组,A组给予PPH联合外痔剥离术的治疗,B组给予传统外切内扎术治疗,比较两组患者的治愈率及平均住院时间,术后严重疼痛、水肿、肛门狭窄等术后并发症的发生率。结果:A组治愈率、平均住院时间、术后并发症的发生率分别为100%、(7.1±0.8)d、14.7%,B组为97.1%、(12.2±1.1)d、70.6%,两组治愈率比较,差异无统计学意义(P>0.05);A组平均住院时间、术后并发症的发生率均显著低于B组,差异有统计学意义(P<0.05)。结论:痔上黏膜环切钉合术联合外痔剥离术与传统外切内扎术相比,疗程较短,术后并发症的发病率较低,患者痛苦较轻,值得临床推广应用。Objective： To compare the clinical treatment between PPH combined with external hemorrhoids dissection and routine procedure of external resection and internal ligation for acute incarcerated circumferential mixed hemorrhoids. Methods： 68 cases of acute incarcerated circumferential mixed hemorrhoids patients were randomly divided into group A and group B. Group A was given the treatment of PPH Combined with dissection and resection for external hemorrhoids, group B was given routine procedure of external resection and internal ligation. Results： Cure rate, average length of stay, incidence of postoperative complications of group A was 100%, （7.1 ±0.8） days, 14.7%, that of Group B was 97.1%, （12.2± 1.1） days, 70.6%. Conclusion： Compared with the routine procedure of external resection and internal ligation, PPH combined with external hemorrhoids dissection surgery results in a shorter course of treatment, a lower incidence of postoperative complications, and the patients suffer less. It is worthy of clinical application.
