获得性免疫缺陷综合征合并马红球菌肺炎的胸部影像学表现

摘　　要：目的探讨获得性免疫缺陷综合征(AIDS)合并马红球菌肺炎患者的胸部影像学表现。方法回顾性分析确诊的15例AIDS合并马红球菌肺炎患者的胸部影像学资料。结果本组15例患者影像学表现如下:①12例(12/15,80.00%)病灶呈结节型,1例为单发结节,11例为多发结节;②2例(2/15,13.33%)病灶呈肿块型,其中1例约10.0cm×12.0cm,边缘光滑,另1例约3.7cm×4.1cm,边缘模糊;③1例(1/15,6.67%)病灶呈斑片型。15例中,13例(13/15,86.67%)病灶内出现空洞;9例(9/15,60.00%)合并少量胸腔积液或胸膜肥厚;2例(2/15,13.33%)合并纵隔淋巴结肿大。治疗后随访3～26个月,1例病灶完全吸收,11例明显吸收,1例失访,2例死亡。结论 AIDS合并马红球菌肺炎常表现为单侧或双侧肺野多发结节影,发现巨大肿块影可能对诊断具有提示意义。Objective To observe the chest imaging features of acquired immunodeficiency syndrome （AIDS） combined with Rhodococcus equi pneumonia. Methods Chest images of 15 patients with AIDS combined with Rhodococcus equi pneumonia were retrospectively analyzed. Results Imaging features of AIDS combined with Rhodococcus equi pneumonia were as follows： （1） Nodules were detected in 12 cases （12/15, 80.00 %）, including single nodule in 1 and multiple nodules in 11 patients. （2）Single mass was found in 2 cases （2/15, 13.33%）, including 1 of 10.0 cm×12.0 cm with smooth edge, another 1 of 3.7 cm ×4.1 cm with blur edge. （3）Patehes were found in 1 case （1/15, 6.67%）. Cavities were found in 13 cases （13/15, 86.67%）. Small amount of pleural effusion or pleural thickening was noticed in 9 eases （9/15, 60.00%）, while mediastinal lymph nodes enlargement was found in 2 cases （2/15, 13.33%）. During following up of 3--26 months, lesions in one case were completely absorbed, in 11 cases were significantly absorbed. One patient was lost during following up, and 2 patients died. Conclusion Multiple nodules in one lateral or bilateral lungs are common imaging findings of AIDS combined with Rhodoeoecus equi pneumonia. Detecting single huge mass may be important for the diagnosis of this disease.
