摘  要:目的探讨乳腺癌术后局部复发的超声表现特点。方法回顾性分析2010年5-11月在重庆市肿瘤医院超声科检查并经病理证实的63例乳腺癌根治术或改良根治术后局部复发的患者的临床资料和超声声像图表现,并对其超声二维声像图特征及彩色多普勒血流特点进行总结。结果超声诊断符合率为95%。2例误诊为胸壁术后瘢痕,1例误诊为胸壁淋巴管炎。局部复发灶二维声像图表现分为3型:不规则肿块型(75%)、均质性结节型(21%)、致密浸润型(4%)。彩色多普勒血流特点:81个复发灶血流检出率为86%,68%为Ⅰ级,18%为Ⅱ级。Ⅰ~Ⅱ级病灶内的动脉血流频谱,PSV:(9.13±2.26)cm/s,RI:(0.60±0.07)。结论超声对早期发现乳腺癌术后局部复发灶有一定价值,能为临床提供较准确的信息。Objective To investigate the uhrasonographic features of locally recurrent breast cancer after mastectomy. Methods Clinical data and uhrasonographic features in the 63 cases with pathologically confirmed local recurrence after mastectomy, who examined in the department of uhrasonography in Chongqing Cancer Institute from May to November in 2010 were retrospectively analyzed. And the features of the two- dimensional uhrasonography and the color Doppler flow imaging were summarized. Results The accurate diagnostic rate was 95 %. Two cases were misdiagnosed as chest wall scar, and one case as chest wall lymphan- gitis. Two-dimensional uhrasonographic features of these cases were divided into 3 types: irregular mass type (75%) , homogeneous nodular type (21%) , and dense infiltrating type (4%). Color Doppler flow showed that blood flow detection rate of 81 tumor lesions was 86% , and 68% of them were in grade I , and 18% in grade Ⅱ The arterial flow spectrum in the grade I and I/ lesions were 9.13 ± 2.26 cm/s for peak systolic velocity, and (0.60 ± 0.07) for resistance index. Conclusion Ultrasonography is of certain value for early detection of breast cancer local recurrence, and provides accurate information for clinical diagnosis
