摘 要:目的评价同一临床治疗模式下,SSRIs对首发抑郁症患者社会功能的改善作用。方法对符合要求的124例首发抑郁症患者,接受患者教育课程后随机给予4种SSRIs抗抑郁药物,分别在治疗前及治疗后第6、12周末采用汉密尔顿抑郁量表(HAMD)进行症状评估,WSAS和SF-12量表进行社会功能评估。结果共有81例患者完成整个随访研究,其中79例达到有效标准,76例达到临床治愈标准,无效2例。SF-12评分较治疗前PF、RP、SF、RE维度得分显著升高,BP、GH、VT维度得分显著下降,总分、PCS和MCS显著升高,其差异均具有统计学意义(P均〈0.05)。WSAS各条目分和总分评分治疗前后差异均有统计学意义(P均〈0.05)。结论在同一临床治疗模式下,临床一线SSRIs治疗首发抑郁症,临床疗效显著,社会功能方面,除SF-12评分中BP、GH、VT三个维度外,总的来说随着治疗时间的延长逐渐改善,有利于患者适应社会。Objective To investigate the effect of standardized treatment on social function in first -episode depression. Methods A total of 124 consenting adult patients with confirmed DSM-IV diagnosis of depressive disorder received standardized education course were brought into an randomized therapy with SSRIs. The therapeutic efficacy was measured by Hamilton Depression Scale(HAMD) and the improvement of social function is measured by Work and Social Adjustment Scales (WSAS) and SF-12 at the end of week 6 and week 12. Results 81 patients completed the i2 weeks' trial. 79 patients responded to the treatment,and in which 76 patients even met the criteria of cure and 2 invalid. There were significant decreases in the score of HAMD-17, the total score, and the score of each item of WSAS after treatment. After standardized treatment, the score of PF, RP, SF and RE in SF- 12 were increased significantly. On the contrary, the score of BP,GH,VT in SF-12 were decreased significantly. Conclusions SSRIs had a significant effect in first-episode depression under the mode of standardized clinic treatment. And the social function was improved as the therapy time prolongs.
