纤维支气管镜应用于烧伤合并吸入性损伤早期的诊疗观察

摘　　要：目的:观察纤维支气管镜在烧伤合并中度以上吸入性损伤患者早期的诊疗效果。方法:2009年1月至2011年6月收治的特重度烧伤合并中度以上吸入性损伤患者56例,均通过纤维支气管镜检查以了解气道损伤情况;清除气道内痰栓、坏死脱落的黏膜组织;局部注药。比较纤维支气管镜治疗前后30 min～1 d患者心率、呼吸频率及动脉血气分析的变化。结果:纤维支气管镜可以确定吸入性损伤的部位以及气道黏膜的损伤程度,纤维支气管镜治疗后,患者的心率、呼吸频率减慢,而动脉血氧饱和度、动脉氧分压较治疗前升高。结论:纤维支气管镜在中度以上吸入性损伤患者的早期气道损伤诊断和辅助治疗方面具有重要作用。Objective To observe the treatment efficiency of fiberoptic bronchoscopy for burnt patients complicated by moderate or serious inhalation injury.Methods Fifty-six burnt patients with moderate or serious inhalation injury were examined by fiberoptic bronchoscopy.The sputum bolt and necrotic mucosa were removed from the airway,and local injection was performed.The heart rate,respiratory rate and changes in blood gas analysis of these patients were monitored 30 minutes and a day before and after treatment.Results With the fiberoptic bronchoscopy,the inhalation injury site,injury degree could be determined,and the heart rate and respiratory rate were decreased,and the arterial oxygen saturation and arterial oxygen pressure were increased.Conclusion Fiberoptic bronchoscopy can be effective and important in early diagnosis and treatment of moderate or severe inhalation injury.[Chinese Medical Equipment Journal,2012,33（6）：57-58,68]
