肾动脉狭窄的CE-MRA和CTA的对照性研究

摘　　要：目的:对肾动脉的三维动态增强磁共振血管造影(CE-MRA)和CT血管成像(CTA)进行对照性研究,评价CE-MRA与CTA这2种不同的检查方法对肾动脉狭窄性病变的显示情况。方法:对临床上怀疑有肾动脉狭窄的30例患者同时行肾动脉的CE-MRA与CTA检查,其中证实有狭窄的15例患者再行数字减影血管造影(DSA)检查,对比分析CE-MRA与CTA这2种不同的影像学检查方法对肾动脉狭窄的显示情况。结果:CE-MRA与CTA在肾动脉狭窄程度的判定上没有明显的区别(P>0.05),CE-MRA在肾动脉狭窄诊断上的敏感性、特异性、阳性预测值、阴性预测值要略低于CTA。结论:CE-MRA与CTA都能较准确地诊断肾动脉的狭窄,但CE-MRA无辐射、更安全,更应作为临床上检查肾动脉狭窄的首选方法。Objective To assess the value of the three-dimensional dynamic contrast-enhanced MR angiography（CEMRA） and CT angiography（CTA） in the diagnosis of renal artery stenosis.Methods Thirty consecutive patients suspected of renal artery stenosis were studied with both CE-MRA and CTA,and fifteen patients confirmed with stenosis underwent DSA at the same time.Then comparative analysis of CE-MRA and CTA imaging on the display of renal artery stenosis was performed.Results There was no significant difference to determine renal artery stenosis between CE-MRA and CTA（P0.05）.In the diagnosis of renal artery stenosis,the sensitivity,specificity,positive predictive value,negative predictive value of CE-MRA was slightly lower than CTA.Conclusions With the same diagnosis value as CTA,CE-MRA,being nonradiative and safe,is more appropriate for the diagnosis of the renal artery stenosis.[Chinese Medical Equipment Journal,2012,33（6）：65-66]
