全髋关节和双极人工股骨头置换术用于老年股骨颈骨折治疗的对比观察

摘　　要：目的对比全髋关节和双极人工股骨头置换术在治疗老年股骨颈骨折临床疗效上的区别。方法将2006～2007年收治的114例老年股骨颈骨折患者随机平均分为两组,治疗组57例,采用全髋关节置换术;对照组57例采用双极人工股骨头置换术。记录两组患者手术时间,术中出血量,24引流量,并对两组患者于术后第1、3、5年进行随访,评估术后疗效。结果两种方法在手术时间、术后出血量、24h引流量及术后第1年疗效等方面比较差异无统计学意义(P>0.05)。在术后第3、5年,治疗组患者疗效及优良率均优于对照组(P<0.05)。结论两种方法均具有手术时间短,术后出血少,术后早期疗效好等优点,但全髋关节置换术在远期疗效上优于双极人工股骨头置换术。Objective To compare the effects of bipolar prosthetic replacement and total hip arthroplasty in treatment of the fracture of femoral neck in the elderly. Methods the 114 patients of the fracture of the femoral neck from ,2006 to 2007 in our hospital for treatment were divided into 2 groups randomly.The treatment group of 57 patients uses Total Hip Replacement when the control group takes the Bipolar prosthesis replacement. Keep notes of the operation time, postoperative blood loss, the amount of bile drainage in 24h of all the patients.and take the follow-up visit of them in the lst, 3rd, 5th year after the surgery ,estimate the effect after the surgery. Results Both of two means is not comparable in the operation time, postoperative blood loss, the amount of bile drainage in 24h and 1 year after the curative effect （P〉0.05）. The effect of surgery and the excellence rate in the treatment group is better than the control group in the 3rd and 5th year after the surgery. Conclusion Both kinds of the surgery is advantage in the operation time, postoperative blood loss and 1 year after the curative effect, but Total Hip Replacement has a better effect in the longterm outcomes than the Bipolar prosthesis replacement.
