复合皮移植治疗功能部位烧伤临床疗效分析

摘　　要：目的探讨脱细胞异体真皮与自体刃厚皮片的复合移植治疗功能部位深度烧伤的应用及疗效。方法 40例深度烧伤患者1周内一次性行切(削)痂术,去除坏死的皮肤及间生态组织,移植大张脱细胞异体真皮,然后切取大张自体刃厚皮0.10～0.25mm覆盖于其上。结果本组40例患者中全部植皮成功,植皮成活率为100%,随访30～36个月,结果发现40例患者脱细胞异体真皮无排异现象,外观和质地基本接近正常皮肤。瘢痕增生不明显,关节活动无明显受限。结论脱细胞异体真皮与自体刃厚皮片复合移植术修复后,外观平整光滑,瘢痕增生不明显,功能活动良好,是深度烧伤创面良好修复材料。Objective To discuss the application and effect of composite skin graft of acellular allogenic dermis and autologous split-thickness skin in the treatment of deep-bums in the function regions. Methods Within lweek after the deep-bums, took the eschar shaving one time to the superficial fascia, vulsed the skin and then completely removed the necrotic skin and in-depth inter-eco tissue. Grafted the large-piece of acellular allogenic dermis and autologous split-thickness skin and cut the large-piece of autologous split-thickness skin 0.10-0.25mm to cover it. Results After 12-18 months of observation, there was no rejection phenomenon for the acellular allogenic dermis, with the similar appearance and texture to the normal skin. There was no significant scar hyperplasia or obvious limitations of joint movement. Conclusion After the composite skin graft of acellular alloganic dermis and autologous split-thickness skin, the skin has a smooth and even appearance, without the significant scar hyperplasia. The function activity is well. It is the good material for the reoair of deep bum wounds.
