糖尿病胆囊运动功能的观察

摘　　要：目的 :观察胆囊运动功能及其与糖尿病程之间的关系。方法 :糖尿病患者 36例 ,分为 A组 (病程大于 5年 ) 18例 ,B组 (病程 5年以下 ) 18例 ,及 C组 (对照组 ) 2 0例。 B超测定空腹和脂肪餐后胆囊容量 ,计算胆囊排空指数 ,比较各组间的差异。结果 :空腹和餐后胆囊容量 ,A组 (2 0 .6 7± 5.6 9ml和 11.6 1± 4 .93ml)与对照组 (17.0 3± 3.2 2和 6 .6 1± 2 .15ml)比较 ,P<0 .0 2和 P<0 .0 0 1。B组 (19.96± 4 .96 ml和 8.56± 2 .77ml)与对照组比较 ,P<0 .0 5和 P<0 .0 2。 A,B两组间比较无差异 ,P>0 .0 5。胆囊排空指数 (gallbladder ejection fraction,GBEF) ,A组 (4 5%± 10 .0 8% )与对照组 (6 1.72 %±7.75% )比较 ,P<0 .0 0 1。 B组 (57.37%± 7.15% )与对照组比较无差异。 A,B两组间比较 P<0 .0 0 1)。结论 :糖尿病患者胆囊容量增加 ,GBEF降低 ,尤以病程超过 5年组明显。胆囊运动功能障碍的程度与糖尿病病程呈正相关 ,自主神经病变可能为重要的发病机制之一。Objective:[WT5BZ][ST5BZ]To observe the correlation of the gallbladder motility dysfunction in diabetics with the duration of diabetes.[WT5HZ]Methods:[WT5BZ]36 diabetics were divided into two groups,group A(n=18,duration of diabetes>5years),group B(n=18,duration of diabetes≤5 years),and group C(n=20,controls).Gallbladder measurement was performed by one sonographer using real time ultrasonography at fasting and 30 min after a fattymeal,and GBEF was calculated.The results were compared.[WT5HZ]Results:[WT5BZ]The volume of the gallbladder during fasting and after a fatty meal,group A(20.67±5.69mland 11.61±4.93ml) was compared with the control group (17.03±3.22mland 6.61±2.15ml),P<0.02and P<0.001.Group B(19.96±4.96ml and 8.56±2.77ml)was compared with the control group,P<0.05 and P<0.02.No difference was observed between group A and group B.P>0.05.GBEF,group A(45%±10.08%)was compared with the control group(61.72%±7.75%),P<0.001).No difference was observed between group B and the control group.However,there was significant difference between group A and group B,(P<0.001).[WT5HZ]Conclusion:[WT5,5”BZ]callbladder volume in diabetics increases and GBEF reduces,which is especially outstanding in the diabetics with over 5 years duration of diabetes.The extent of gallbladder motility dysfunction is positively correlation with the duration of diabetes.It is feasible that autonomic neuropathy is one of the important mechanisms.
