超声心动图评价高效抗逆转录病毒治疗艾滋病的心血管安全性

摘　　要：目的 探讨应用超声心动图评价高效抗逆转录病毒治疗艾滋病的心血管安全性。方法 应用超声心动图测量45例艾滋病患者高效抗逆转录病毒治疗前、治疗48周及96周心脏各房室腔内径、室壁厚度、瓣膜形态、瓣口血流速度、室壁运动速度、左心室缩短分数、射血分数。结果 45例艾滋病患者治疗前与治疗96周间、治疗48周与治疗96周间的左室流出道血流速度（V-LVOT）［（115.0±13.8）cm/s比（107.0±13.3）cm/s,（113.0±12.9）cm/s比（107.0±13.3）cm/s］、二尖瓣口E峰减速时间（EDT）［（166.8±26.7）ms比（140.4±19.3）ms,（172.9±31.0）ms比（140.4±19.3）ms］,治疗前［（26.8±6.7）cm/s］与治疗48周［（23.8±5.0）cm/s］、治疗前与治疗96周间（22.7±6.4）cm/s的二尖瓣环处舒张早期运动速度（E′）差异均有统计学意义（均P〈0.05）,其余各项测值间差异均无统计学意义（均P〉0.05）。结论 高效抗逆转录病毒治疗艾滋病短期内具备心血管安全性,超声心动图可准确评价抗病毒治疗后心脏功能。Objective To explore the clinical value of echocardiography in evaluating the cardiovascular safety of drugs for AIDS patients treated by highly active antiretroviral therapy（HAART）.Methods 45 HIV-infected patients accepted three times of echocardiography respectively before the treatment,and after 48 weeks of HAART and 96 weeks of HAART.All kinds of echocardiographic parameters were measured.Results There were statistically significant differences among baseline group pretreatment［V-LVOT：（115.0±13.8）cm/s,EDT：（166.8±26.7）cm/s,E′：（26.8±6.7）cm/s］,48 weeks ［V-LVOT：（113.0±12.9）cm/s,EDT：（172.9±31.0）cm/s,E′：（23.8±5.0）cm/s］ of HAART and 96 weeks［V-LVOT：（107.0±13.3）cm/s,EDT：（140.4±19.3）cm/s,E′：（22.7±6.4）cm/s］ of HAART for parameters of V-LVOT,EDT and E′（all P〈0.05）,other echocardiographic parameters showed no statistically significant differences（all P〉0.05）.Conclusion Short-term of HAART was provided with cardiovascular safety,the invasive echocardiography could accurately evaluate the cardiac function and structure of AIDS patients with HAART.
