孕妇妊娠期D-二聚体和凝血指标的变化

摘　　要：目的探讨孕妇妊娠期D-二聚体(D-D)和凝血指标的变化。方法测定30例正常非孕育龄妇女(对照组)、60例正常孕妇(早、中、晚期孕妇各20例)和30例合并妊娠高血压孕妇的血浆D-D和凝血酶原时间(PT)、部分凝血活酶时间(APTT)及纤维蛋白原(Fib)含量,比较其组间差异。结果妊娠高血压组PT为(10.6±1.0)s,明显低于晚孕组的(11.4±0.8)s和中孕组的(11.9±0.7)s(P<0.05);妊娠高血压组APTT为(28.4±2.2)s,明显低于中孕组的(31.2±2.2)s(P<0.05)。妊娠高血压组Fib为(5.1±1.0)g/L,明显高于晚孕组的(4.2±0.9)g/L和中孕组的(3.8±0.6)g/L(P<0.05);妊娠高血压组D-D为(0.72±0.4)mg/L,明显高于晚孕组的(0.48±0.2)mg/L和中孕组的(0.28±0.2)mg/L(P<0.05)。上述测定值均与早孕组有统计学差异(P<0.05)。结论对孕妇,尤其是合并妊娠高血压者监测D-D和凝血三项指标的变化有助于凝血与纤溶功能的评估。Objective To investigate the Changes of D-dimer（D-D） and blood coagulation index during pregnancy. Methods The D-D and three blood coagulation indexes of prothrombin time（PT）, activated partial thromhoplastin time （APTT） and fibrogen （Fib） were examined in 30 non-pregnant women（group C）,60 normal pregnant women in the early（group B1,20 cases）, middle（group B2,20 cases） and late pregnancy（group B3,20 cases）, and 30 pregnant women with gestational hypertension. The differences of D-D and three blood coagulation indexes were compared among 5 groups. Results Plasma PT of group A was （10. 6±1.0） s,which was less than （11.4±0.8） s in group 133 or （11.9±0. 7） s in group B2（P〈0. 05）. Plasma APTT of group A was （28. 4±2. 2） s,which was less than （31.2 ±2. 2） s in group B2（P〈0. 05）. Fib of group A was （5.1±1.0） g/L,which was higher than （4. 2± 0. 9） g/L in group 133 or （3.8-±-0.6） g/L in group B2（P〈0. 05）. Plasma IND of group A was （0. 72 ±0. 4） mg/L,which was higher than （0.48±0. 2） mg/L in group B3 or （0. 28±0.2） mg/L in group B2（P〈0. 05）. The above values were all significantly different from those in group B1 （P〈0. 05）. Conclusion Monitoring plasma D-D and above three blood coagulation indexes is helpful in evaluating the coagulation and fibrolysis status during pregnancy, especially in those with gestational hypertension.
