马来酸曲美布汀联合六味安消和(或)氟哌噻吨美利曲辛治疗糖尿病胃轻瘫的疗效

摘　　要：目的观察马来酸曲美布汀联合六味安消和(或)氟哌噻吨美利曲辛治疗糖尿病胃轻瘫(DGP)的有效性和安全性。方法 151例DGP患者随机分为四组:马来酸曲美布汀组(A组,36例);马来酸曲美布汀+六味安消组(B组,41例);马来酸曲美布汀+氟哌噻吨美利曲辛组(C组,35例);马来酸曲美布汀+六味安消+氟哌噻吨美利曲辛组(D组,39例)。治疗20d后,观察患者的消化道症状改善情况及不良反应。结果 A、B、C、D组治疗后恶心、呕吐症状减轻的有效率分别为61.11%、68.29%、45.71%、71.79%,治疗后腹痛腹胀症状减轻的有效率分别为47.22%、70.73%、62.86%、92.30%,治疗后大便性状明显好转的有效率分别为50.00%、78.05%、48.57%、82.05%。结论单用马来酸曲美布汀适合治疗轻、中度DPG患者,联合六味安消组成的二联适合治疗较重的DPG患者,再加上氟哌噻吨美利曲辛组成的三联治疗DGP最有效,特别适合重型DPG患者。Objective To investigate the efficacy and safety of trimebutine maleate combined with liuwei anxiao and/or flupentixol and melitracen in the treatment of diabetic gastroparesis（DPG）. Methods A total of 151 consecutive outpatients with DGP was randomly divided into four groups of A（36 cases, treated with trimebutine maleate）, B（41 cases, treated with trimebutine maleate combined with liuwei anxiao）, C（35 cases, treated with trimebutine maleateand combined with flupentixol and melitracen） and D （39 cases, treated with trimebutine maleate combined with liuwei anxiao and flupentixol and melitracen）. Twenty days after treatment, the improvements of gaistrointestinal symptoms and adverse reactions were observed and recorded. Results Total effectiveness rates of attenuating nausea and vomiting in groups of A,B,C and D were 61.11%, 68. 29%,45.71% and 71.79%, respectively. Total effectiveness rates of attenuating abdominal pain and distension in groups of A, B,C and D were 47.22 %, 70.73%, 62.86% and 92.30%, respectively. Total effectiveness rates of attenuating stool status in groups of A, B, C and D were 50. 00 %, 78. 05 %, 48.57 % and 82.05 %, respectively. Conclusion Trimebutine maleate is suitable for treating the patients with mild DGP. In the patients with moderate DGP, trimebutine maleate combined with liuwei anxiao is a better choice. For the patients with severe DGP, trimebutine maleate combined with liuwei anxiao and flupentixol and melitracen is suggested.
