妊娠早期孕妇血清中ADAM12-S水平与异位妊娠的相关性研究

摘　　要：目的:探讨妊娠早期孕妇血清中ADAM12-S水平变化与异位妊娠的关系。方法:选取2010年7月至2011年5月就诊于我院的60例异位妊娠孕妇(研究组),120例正常单胎妊娠孕妇(对照组),于5～9孕周应用时间分辨荧光免疫分析法检测其血清中ADAM12-S水平,并分析ADAM12-S水平与异位妊娠的关系。结果:①120例正常对照组孕妇5、6、7、8、9孕周血清中AD-AM12-S水平分别为6.80±0.56、17.45±1.22、40.24±2.72、88.64±5.31、162.42±15.78μg/L,总体趋势是随孕周的增加而增加,且明显的线性关系,相关系数(r)为0.898(P<0.05)。②60例研究组孕妇血清ADAM12-S水平一直处于低值状态,且随着孕周的增加变化不明显,甚至有所降低,未呈现线性关系,r为0.064(P>0.05)。③研究组各孕周的中位数倍数明显低于正常对照组,差异有高度统计学意义(P<0.01)。结论:异位妊娠孕妇血清ADAM12-S水平明显低于正常单胎妊娠者,可以作为异位妊娠早期诊断的指标之一,检测血清ADAM12-S水平可提高异位妊娠的早期诊断率。Objective：To explore the possible association of maternal serum levels of disintegrin and met- alloprotease （ADAM12-S） and ectopic pregnancy in the first trimester of pregnancy. Methods： The mater- nal serum concentration of ADAM12-S from 5 to 9 weeks was measured in 60 cases of ectopic pregnancy and 120 controls matched to the cases by gestational age and length of storage in our hospital from July 2010 to May 2011. ADAM12-S levels were measured by the automated AutoDELFIA immunoassay platform. The relationship between ADAM12-S level and ectopic pregnancy were analyzed. Results：The median lev- els of ADAM12-S at 5,6,7, 8,9weeks of gestation were 6.80±0.56μg/L, 1.7. 45±1.22 μg/L, 40.24±2.72 μg/L, 88.64±5.31μg/L, 162.42±15.75 μg/L respectively, showing a linear increase with the gesta- tional age （r=0. 898, P 〈0.05）. In the control group the concentration of ADAM12-S increased with fetal crown rump length. The multiple of medians （MOMS） of ectopic pregnancy group was significantly lower than those of the control group （P 〈0. 01 ）. Conclusions：There is a good correlation between the maternal serum ADAM12-S and ectopic pregnancy. Measurement of ADAM12-S in the first-trimester provides useful prediction of ectopic pregnancy.
