三种内固定方式治疗股骨粗隆间骨折的并发症原因分析

摘　　要：目的:探讨老年股骨粗隆间骨折内固定术后并发症的原因和治疗对策。方法:回顾总结我院2004年1月～2010年9月179例股骨粗隆间骨折的病例,分析术后并发症的原因。结果:145例获得随访,13例出现术后并发症,发生率8.97%。术后并发症包括单纯髋内翻8例,股骨头颈钉切割穿入髋臼合并髋内翻3例,螺钉断裂1例,股骨颈短缩1例。结论:髋部生物力学的特殊性及老年人骨质疏松增加了术后并发症的概率。复位欠佳、内固定物选择不当、手术操作技术欠佳和不恰当的康复锻练是发生术后并发症的主要原因。Objective：To explore the causes and countermeasures of complications after internal fixation of intertrochanteric fractures.Methods：Reviewed and analyzed the causes of complications after surgery in 179 patients with femoral intertrochanteric fracture from January 2004 to September 2010 in our hospital.Results：145 patients were followed up for 6 to 36 months（22.3 months averagely）.The complications were occurred in 13 patients（the rate was 8.97%）.The postoperative complications included eight cases of pure coxa vara,three cases of coxa vara accompanied with femoral neck nail cutting through the acetabulum,one case of screw breakage,and one case of lower limbs shorten.Conclusion：The particularity of hip biomechanics and elderly osteoporosis increases the probability of postoperative complications.Reset poor fixation,improper implant selection,poor surgical technique,and appropriate rehabilitation exercise are the main reasons for postoperative complications.
