摘 要:目的通过人工控制灌注压力,减少输尿管镜下钬激光治疗输尿管上段结石时结石逃逸率。方法对比人工手控液压灌注和液压灌注泵灌注时术中碎石成功率、结石逃逸率。结果人工手控灌注时碎石成功率为94.02%,结石逃逸率为5.98%;液压灌注泵灌注时碎石成功率为80.7%,结石逃逸率为19.3%。人工手控灌注时碎石成功率显著高于灌注泵灌注。结论输尿管镜钬激光治疗输尿管上段结石时采用人工手控液压灌注可有效减少结石逃逸率,增加碎石成功率。Objective To explore the influence of artificial hand controlled infusion on the successful rate of stone frag- mentation and escape rate of stone in upper ureteral calculi transurethral ureteroscopy holmium laser lithotripsy. Methods 429 patients underwent endoscopic lithotripsy either with the artificial hand controlled infusion (228 cases) or with hydraulic infusion pump infusion(201 eases) for the treatment of ureteral stones. Results The overall successful rate of stone fragmentation of ar- tificial hand controlled infusion group was higher than that of hydraulic infusion pump infusion group (94.02% vs 80.7%, P〈0.05) ,While the stone escape rate was lower (5.98% vs 19.8% ,P〈0.05). Conclusion Artificial hand controlled infusion can improve the successful rate of stone fragmentation,while reducing the escape rate in upper ureteral calculi transurethral ureteroscopy holmium laser lithotripsy.
