双侧解剖钢板治疗胫骨平台骨折的效果观察

摘　　要：目的探讨胫骨平台骨折采用双侧解剖钢板治疗的临床效果。方法选择我院2009年4月至2011年4月收治的胫骨平台骨折患者70例,随机分为两组,观察组35例采用双铡解剖钢板治疗,对照组35例采用单纯T形钢板内固定术,就两组临床资料进行回顾性分析。结果观察组35例患者中,伤口均Ⅰ期愈合,无神经血管损伤、皮肤坏死、深部感染等并发症的发生,术后15.5w骨折愈合,患膝骨性稳定和正常力线均恢复,无内固定断裂及松动;行平均12个月的随访,优良率为97.1%。对照组35例中,深部感染1例,内固定松动及断裂各1例,优良率为80%。观察组优良率显著高于对照组,并发症发生率低于对照组,差异均有统计学意义(P<0.05)。结论胫骨平台骨折采用双侧解剖钢板治疗,可提高治疗效果,降低并发症发生率,使患者生活质量明显提高。Objective To explore the clinical curative effect of bilateral anatomical plate in the treatment of tibial plateau fractures. Methods 70 patients with tibial plateau fractures in our hospital from April 2009 to April 2011 were randomly divided into two groups. The observation group received the treatment of bilateral anatomical plate, while the control group received the treatment of internal fixation by T-shaped plate alone. The clinical data of two groups were analyzed retrospectively. Results Among 35 cases in the observation group： all wounds were healed in Ⅰ phase without neurovascular injury, skin necrosis, deep infection or other complications; the fracture healing occured 15.5 w after the treatment; all patients obtained bone stability and normal lines recovery and no internal fixation breakdown or loose was found; the results of average 12 months of follow-up showed the excellent and good rate of 97.1%. Among 35 cases in the control group, the results showed one case of deep infection, one case of internal fixation breakdown, one case of loose internal fixation and the excellent and good rate of 80%. The observation group had significantly higher excellent and good rate and significantly lower the incidence of complications compared with the control group, the differences were statistically significant （P 0.05）. Conclusions Bilateral anatomical plate in the treatment of tibial plateau fractures can improve the curative effect, reduce the incidence of complications and improve patients＇ quality of life.
