摘 要:目的:了解武汉市机关和企事业单位人群脉搏波速度(PWV)和踝臂指数(ABI)水平,并探索影响因素。方法:以来自武汉市机关和企事业单位的1 551例体检人群为研究对象。检测项目包括:PWV、ABI、体质指数(BMI)、总胆固醇(TCH)、低密度脂蛋白(LDL)、高密度脂蛋白(HDL)、甘油三酯(TG)、尿酸(UA)、糖化血红蛋白(HbA1)、空腹血糖(GLU)、血压(BP)。应用SPSS 13.0统计软件进行分析,χ2检验用于比较不同组PWV、ABI异常检出率,多元Logistic回归模型用于分析PWV、ABI的相关影响因素。结果:该体检人群PWV异常检测率为42.0%,ABI异常检出率为1.6%。年龄大(40岁以上44.6%,60岁以上69.1%),超重或肥胖(48.0%或46.7%),高血压(69.6%),TCH(50.0%)、LDL(48.9%)、TG(55.0%)、UA(49.4%)和/或GLU(69.4%)检测异常者PWV异常检出率高;男性PWV异常检出率(39.5%)低于女性(46.7%)。肥胖者ABI异常检出率(8.9%)高于非肥胖者。与PWV相关的因素包括年龄、性别、BMI、高血压、TG、UA、GLU;与ABI相关的因素是BMI。结论:该体检人群PWV异常检出率较高,并受年龄、性别、BMI、高血压、甘油三酯、尿酸、空腹血糖的影响;而肥胖是影响ABI水平升高的主要因素。Objective: To understand the levels of pulse wave velocity(PWV) and ankle brachial index(ABI) and to explore their influence factors in staff from government,public organizations and enterprises in Wuhan,China.Methods: A total of 1 551 health check examinees were enrolled into this study.The main test items included PWV,ABI,body mass index(BMI),total cholesterol(TCH),low density lipoprotein(LDL),high density lipoprotein(HDL),triglyceride(TG),uric acid(UA),glycosylated hemoglobin(HbA1),fasting blood glucose(GLU),and blood pressure(BP).SPSS 13.0 was used to analyze the data,with Chi-quare test for comparation of abnormal detection rate between different groups and multivariate logistic regression for exploring influence factors of PWV and ABI.Results: The abnormal detection rate of PWV and ABI were 42.0% and 1.6%.The rate of PWV was higher among the subjects who were older(44.6% for those aged more than 40 years old and 69.1% for those aged more than 60 years old),overweight or obese(48.0% or 46.7%) or whose physiological and biochemical indexes including BP(69.6%),TCH(50.0%),LDL(48.9%),TG(55.0%),UA(49.4%) and GLU(69.4%) were abnormal.The abnormal detection rate of PWV in males(39.5%) was lower than that in females(46.7%).Obese people's abnormal detection rate of ABI was higher than other people's.There was no significant difference on the abnormal detection rate of ABI among health examinees with different age,gender or physiological and biochemical test results.Influence factors associated with PWV were age,gender,BMI,BP,TG,UA and GLU.ABI level was associated with BMI.Conclusion: The abnormal detection rate of PWV in health examinees was high,which was affected by age,gender,BMI,BP,TG,UA and GLU.Obesity was a main risk factor associated with ABI.
