气管切开术后缝合固定套管的临床观察

摘　　要：目的探讨气管切开术后缝合固定气管套管的临床疗效。方法将658例行气管切开术患者随机分为试验组和对照组各329例。对照组术后予以常规气管套管固定;试验组术后予以缝合固定气管套管。对2组术后置管并发症情况进行比较。结果试验组皮肤挫伤、套管脱管、切口出血发生率分别为7.3%、0、0.6%,低于对照组的36.5%、10.3%、14.6%,差异均有显著统计学意义(P<0.01)。结论气管切开套管缝线固定能有效地保护颈部皮肤、减轻患者不适和痛苦、减少出血、避免套管脱出,提高了手术安全性,值得临床推广应用。Objective To investigate the effect of suturing and fixing tracheal tube after tracheotomy. Methods 658 cases of tracheotomy with local anesthesia or general anesthesia were Randomly divided into experiment group and control group with 329 in each. Patients in the control group were given conventional tracheal cannula fixed; in postoperative time patients of test group were fixed by sutured trachea cannula. Tube complications in the two groups were compared. Results Skin Contu- sion,casing tube,incision bleeding rate in the test group was 7.3%, O, 0.6% , respectively lower than that of control group （36.5%, 10.3%, 14.6% ）, and the difference was statistically significant （ P 〈 0. 01）. Conclusion Suturing and fixing the tracheal tube is an effective method for protecting the patientt cervical skin, easing the pain, reducing bleeding and avoid the tracheal tube falling out. It is provided with security and favorable clinical value.
