摘  要:目的 :观察慢性肾功能不全各期患者血清甲状腺激素的变化 ,探讨肾脏受损对其影响及临床意义。方法 :5 1例患者被分为代偿期、失代偿期和衰竭期三组。采用放射免疫法检测血清FT3、FT4 、TT3、TT4 、rT3、TSH。结果 :血清FT3、FT4 、TT3、TT4 降低程度随肾功能损害的加重而明显。肾功能失代偿期Ccr与TT3 呈显著正相关 ,Scr与TT3 呈显著负相关 ;衰竭期Ccr与TT3、FT3、TT4 呈显著正相关 ,Scr与TT3、FT3、TT4 呈显著负相关。结论 :血清FT3、TT3、TT4 的变化与肾脏受损害程度密切相关 ,可以作为评估肾脏病变的严重程度。Objective: To investigate the correlation between renal damage and thyroid hormone in patients with chronic renal insufficiency. Methods: 51 cases were divided into three groups---compensation, decompensation and failure. The serum FT 3、FT 4、TT 3、TT 4、rT 3 and TSH levels in all patients were detected by RIA method. Results: The levels of FT 3、FT 4、TT 3、TT 4 were significantly low in patients with serious impairment of renal function (P<0.001). There was a positive correlation between Ccr and TT 3 (r=0.722, P<0.001), a negative correlation between Scr and TT 3(r=-0.842,P<0.001) in decompensatied group. A positive correlation between Ccr and TT 3、FT 3、TT 4 and anegative correlation between Scr and TT 3、FT 3、TT 4 in group Ⅲ were found. Conclusion: There is a close relationship between levels of serum FT 3、TT 3、TT 4 and renal damage. Change of thyroid hormone might be a marker for evaluation in clinical situation, functional compensation and prognosis of patients with renal insufficiency.
