双弹力包扎在尿道下裂术后的应用

摘　　要：目的探讨一种理想的尿道下裂修复术后阴茎包扎的方法。方法将黏膜片或皮瓣上皮面向内包裹支撑管并缝合成管状，形成尿道。术毕清洁皮肤后，以美皮贴覆盖阴茎体全部，并在美皮贴外面直接安放弹力网套，均匀压力包扎。结果所有患者术后敷料固定良好，无一例脱落；术区更换敷料方便、简单，无明显疼痛感，周围皮肤无浸渍及渗出，无血肿、感染发生。早期并发症少。结论采用双弹力包扎法可以在阴茎勃起和疲软状态下为再造尿道提供适当的压力，减少组织间隙的积血和渗尿，有利于提高尿道下裂手术的成功率，适合临床推广应用。Objective To explore an ideal method for hypospadias repair by penile dressing. Met hods The mucosa or skin flap was used to wrap inward the support tube and sutured to form the urethra. The whole penis was wrapped with mepitel after cleaning the skin, elastic net was placed directly outside mepitel with a uniform compression dressing. Results The dressing remained intact in all the cases without falling off. Dressing change was simple and painless without infusion and effusion around skin, hematoma and infection. There were few early complications. Conclusion A double-elastic dressing after hypospadias repair is an ideal method for applying proper pressure to the reconstructed urethra, which can decrease the interstitial hematocele and urine leaking. It is benefit to improve the successful rate of hypospadias repair and it is worth spreading in clinic.
