大剂量沐舒坦治疗重型颅脑损伤气管切开后并发肺部感染临床分析

摘　　要：目的研究大剂量沐舒坦在重度颅脑损伤气管切开后并发肺部感染中的治疗作用。方法选择重型颅脑损伤气管切开后并发肺部感染患者48例,随机分为对照组和治疗组;治疗组予以静脉滴注沐舒坦(德国勃林格殷格翰公司生产)15 mg/(kg.d),对照组静脉滴注2 mg/(kg.d)。两组其他用药均相同。比较两组患者痰液黏稠度及胸部X线片。结果静脉滴注大剂量沐舒坦,可以使肺部痰液易于变得稀薄,痰液更易于咳出和吸出,胸部X线片显示治疗组肺部感染征象控制好于对照组(χ2=5.07,P<0.05)。结论大剂量沐舒坦有助于改善气管切开患者痰液黏稠度,促进排痰,控制肺部感染,缩短疗程。Objective To research the effects of large dose of ambroxol for treaing complicating pulmonary infection after tracheotomy in severe craniocerebral injury.Methods 48 cases of complicating pulmonary infection after tracheotomy in severe craniocerebral injury were selected and randomly divided into the control group and the treatment group.The treatment group was given ambroxol 15 mg/（kg·d）by intravenous drip,while the control group was given ambroxol 2 mg/（kg·d） by intravenous drip.Other medications were same.The sputum stickiness and chest X-ray were compared between the two groups.Results Large dose of ambroxol by intravenous drip could cause the sputum in lung easy to become thin,cough up and be aspirated.Chest X ray plain demonstrated that the lung infection in the treatment group was controlled well compared with the control group（χ2= 5.07,P0.05）.Conclusion The large dose of ambroxol is helpful to the improve the sputum stickiness,promote sputum expectoration,control the lung infection and shorten the treatment course in the patients with tracheotomy.
