宫颈阴道分泌物绒毛膜促性腺激素和甲胎蛋白检测在胎膜早破诊断中的价值

摘　　要：目的　探讨宫颈阴道分泌物绒毛膜促性腺激素 (hCG)和甲胎蛋白 (AFP)检测 ,在胎膜早破诊断中的价值。方法　对 2 0 8例胎膜早破孕妇和 12 0例正常孕妇的宫颈阴道分泌物进行hCG和AFP检测。结果　hCG诊断胎膜早破的准确率 93 3 % ,敏感性 94 2 % ,特异性 91 7% ;AFP诊断胎膜早破的准确率 92 1% ,敏感性89 4% ,特异性 96 7%。二者差异无显著性 (P >0 0 5 )。结论　hCG和AFP对诊断胎膜早破均有较高的准确率、敏感性和特异性。Objective To study the comparative diagnostic value of two amniotic components : hCG and AFP in premature rupture of membranes.Methods 208 cases with ruptured amniotic membranes and 120 cases of term pregnancy took part in our prospective study .The hCG and ATP in the secretion of cervix and vagina were measured by radioimmunoassay and dot immunobinding assay, respectively.Results The accuracy of hCG and AFP for diagnosis of premature rupture of membranes were 93 3% and 92 1%,respectively(P>0 05).The sensitivity were 94 2% and 89 4%(P>0 05). The specificity were 91 7% and 96 7%, respectively (P>0 05).Conclusion It suggests that both hCG and AFP be diagnostic markers for premature rupture of membranes with good specificity, sensivity and accuracy.
