摘 要:目的观察洞庭湖日本血吸虫病流行区居民(实验组)和非疫区居民(对照组)的外周血单核细胞(RBMC)在体外培养条件下,用日本血吸虫成虫可溶性抗原(AWA)和5种重组基因疫苗候选抗原刺激增生时细胞因子的产生情况,并评价这些细胞因子的产生与人体对日本血吸虫感染抗性之间的关系。方法将来自洞庭湖日本血吸虫病洲岛型流行区2个岛(青山和严家山)的67名实验组对象,按其暴露于疫水的程度和血吸虫病粪检结果,分为抗性组(有较高的暴露程度但血吸虫粪卵阴性)、易感组(血吸虫粪卵阳性)和不确定组(有较低程度的暴露且血吸虫粪卵阴性),来自非疫区的11名居民作为对照组。将试验组和对照组对象的外周血单核细胞在血吸虫成虫可溶性抗原和5种重组抗原(副肌球蛋白、28 kDa谷胱甘肽-S-转移酶、脂肪酸结合蛋白、22 kDa表皮膜相关蛋白和 3-磷酸甘油醛脱氢酶)的刺激下培养 5 d,用 ELISA法检测培养上清液中的白细胞介素 5(IL-5),白细胞介素10(IL-10)和γ-干扰素(IFN-γ)的含量。结果试验组对象的PBMC在AWA刺激下产生的 3种细胞因子的水平均高于对照组;但在重组抗原刺激下,只有 IL-10的产生高于对照组;抗性组对象的 PBMC?Objectives To observer the cytokine profiles producted by human peripheral blood mononuclear cells (PBMC) in vitro following stimulation with S. japonicum soluble adult worm antigen (AWA) or five recombinant S. japonicum antigens, and to assess the association between these cytokine production and resistance to S. japonicum infection. Methods According to epidemiological data, 67 observational group people residing in an area endemic for schistosomiasis japonica (exposed group ) were further divided into three groups. The first was resistant group (with high exposure level and negative in stool examination for schistosome egg, n=21 ), the second was susceptible group (positive in stool examination, n=16) and the third was indeterminate group (with lower exposure level and negative in stool examination, n=30). Peripheral blood monomuclear cells from subjects of observational group and individuals residing in non-endemic area (control group) were incubated in the presence of AWA, or five recombinant antigens. Cytokine including interleukin-5 (IL-5), interleukin-10 (IL-10) and interferon-γ (IFN-γ) in supernatants were determined quantitatively by sandwich-ELISA using cytokine-specific monoclonal antibodies. Results Following the stimulation with AWA, PBMCs from individuals of observational group produced significantly higher level of IL-5, IL-10 and IFN-γ when compared with those from the control. When PBMCs were incubated in the presence of one of five recombinant schistosome antigens, there was only significant difference in IL-10 production between observational and control groups. Following the stimulation of TegAg,FABP or GAPDH,PBMCs from individuals of resistant group produced markedly higher levels of IL-10 when compared with those from susceptible group. Following the stimulation of GST, level of IFN-γ in supernatants of PBMCs from resistant individuals were significantly higher than that of PBMCs from subjects of susceptible group.
