iPTH与慢性肾衰竭患者EPO治疗低反应性贫血的关系

摘　　要：目的探讨维持性血液透析患者免疫反应性甲状旁腺激素(iPTH)与EPO治疗低反应性贫血的关系。方法39例EPO治疗的低反应性贫血患者空腹采血测定Hb、Hct、SF、Ca2+、P3+、CRP、Alb以及Scr和iPTH,并对iPTH明显增高组进行干预,同时每2周测定iPTH、Hb/HCT等。结果 39例EPO治疗低反应患者17例(占43.58%),iPTH升高22例(占56.42%),iPTH增高组EPO/Hct比值、磷(P)、钙磷乘积(Ca×P)和iPTH水平均高于iPTH正常组,Hct、Hb低于PTH正常组;对甲状旁腺功能亢进患者进行治疗干预后血iPTH明显下降、Hb、Hct则均有明显回升。结论甲状旁腺激素升高是影响促红细胞生成素疗效的常见且重要因素之一。Objective To investigate the relationship of iPTH and EPO treatment of low response anemia in chronic Kidney falling while mataining dialysis. Methods 39 examples with low response anemia patients are in the blood dialysis class by the blood ipth, Hb, HCT, SF, Ca2＋ and P3＋, CRP, Alb and Ser, intervent the groupin which iPTH is obvious and the higher, At the same time every two weeks to determine the Hb, HCT and Ipth. Results All of 39 cases with EPO treating low re- sponse 17 examples, in which iPTH is high, 22 examples （56.42%） with iPTH normal EPO/Hct, P, CaxP and iPTH is high- er in the iPTH arising group than the normal iPTH group. Hct, Hb is just Opposite. The iPTH is obvious down while treat- ing in hyperparathyroidism, ang Hh, Hct are arising. Conclusion Arising of the iPTH is one of the most important foctors in treatment of EPO.
