玻璃体切除联合眼内填充物术后不同卧位床护理的效果研究

摘　　要：目的观察玻璃体切除联合眼内填充物术后患者应用自制按摩床和普通病床进行体位护理的效果。方法将72例行玻璃体切除联合气体(C3F8)或硅油填充术治疗的复杂性视网膜脱离患者随机分为观察组和对照组,每组36例,观察组患者术后采用按摩床进行俯卧位面向下体位护理,对照组患者术后采用普通病床进行常规俯卧位面向下体位护理。分别记录两组患者术后俯卧位持续时间及高眼压、角膜水肿、视网膜脱离等并发症的发生率,并采用量表评估两组患者的舒适度。结果观察组患者俯卧位持续时间及舒适度评价均优于对照组,差异有极显著意义(P<0.01);观察组术后高眼压、角膜水肿、视网膜脱离并发症的发生率低于对照组,差异有极显著意义(P<0.01)。结论玻璃体切除联合眼内填充物术后患者使用按摩床采用俯卧位面向下体位护理,能够有效延长患者俯卧位持续时间;增加患者舒适度;明显减少患者术后高眼压、角膜水肿、视网膜脱离等并发症的发生,具有临床推广应用价值。Objective To explore the clinical nursing care effects of using massage tables and usual beds on patiens with intraocular tamponade after vitrectomy. Method 72 patients with complicated retinal detachment after vitrectomy with intraocular gas （C3F8） or silicon oil tamponade were randomly divided into two groups, 36 patients in each group respectively. The routine nursing measues with usual beds were used in the control group and the facial support in prone position using massage tables in the observation group. Recording the lying time, the occurrence rates of complications such as high intraocular pressure, corneal edema and retinal detachment. A self-designed questionnaire survey was conducted on the comfort of the patients. Result The lying time and comfort level evaluation in the observation group were superior to those in the control group with significant differences （P〈0.01）. The occurrence rates of adverse reactions such as high intraocular pressure, corneal edema, retinal detachment were lower than those in the control group with significant difference（P〈0.01）. Conclusion The facial support in prone position by massage tables can effectively improve the patients＂ lying time and comfort degree, reduce adverse reactions, this effect has significant clinical implications.
