生肌膏联合德莫林治疗糖尿病足溃疡的实验研究

摘　　要：目的评估生肌膏联合德莫林治疗大鼠糖尿病足溃疡的治疗效果。方法采用腹腔注射链脲佐菌素45mg／kg复制糖尿病大鼠模型；饲养2个月后于大鼠后足背部按Wagner分级3级标准制备烫伤溃疡模型。20只制模后尾静脉血糖〉16mmol／L的糖尿病足溃疡模型大鼠按随机数字表法分为实验组和对照组，每组10只，两组在治疗的前3d均给予生肌膏，实验组5d后改用德莫林。治疗3、7、11、15d测量溃疡愈合情况。结果两组治疗后溃疡面积（cm^2）均随治疗时间延长呈明显减小的趋势，与前一时间点比较，差异均有统计学意义（均P〈0．01）；治疗3d、7d对照组与实验组比较差异无统计学意义（3d：0．540±0．035比0．559±0．013，7d：0．460±0．018比0．439±0．035，均P〉0．05），治疗11d、15d实验组较对照组缩小更显著（11d：0．350±0．035比0．380±0．025，15d：0．240±0．025比0．310±0．026，P〈0．05和P〈0．01）。结论在溃疡后期联合应用生肌膏和德莫林对加快糖尿病足溃疡愈合有一定的促进作用。Objective To evaluate the effect of demoli dressing stick combined with myogenic ointment of traditional Chinese medicine on ulcer healing of rat diabetic foot. Methods Rats were injected intraperitoneally with streptozocin 45 mg/kg once to induce models of diabetes. After 2 months of high fat feeding, according to the level of grade 3 in the Wagner classification, the posterior foot dorsal skin of rat was scalded to prepare an ulcer. Twenty model rats with diabetic foot ulcer and sugar level 〉16 mmol/L in blood collected from a caudal vein were randomly divided into a control group and an experiment group （each n=10）. On the 1st day and 3rd day, myogenic ointment was applied onto the ulcer in the two groups ; after the 5th day, demolin dressing was added in the experiment group, then the size of ulcers were measured on the 3rd, 7th, llth, 15th days in the two groups. Results On the 7th day, the ulcer＇s size （cm^2 ） was obviously smaller than that on the 3rd day in the two groups （both P〈0.01 ）, however, the size comparison between the two groups on the 3rd and 7th days had no statistical significance （the 3rd day ： 0.540±0.035 vs. 0.559±0.013, the 7th day： 0.460±0.018 vs. 0.439±0.035, both P〉0.05）, but on the llth and 15th days, the ulcer size in the experiment group was significantly smaller （the llth day ： 0.350±0.035 vs. 0.380±0.025, the 15th day ： 0.240±0.025 vs. 0.310±0.026, P〈0.05 and P〈0.01）. Conclusion The combined application of the ointment and demoli has certain therapeutic effect on diabetic foot ulcer at the late phase.
