摘 要:目的:探讨利用混合呼出气氧浓度(expired oxygen concentraion,FEO2)估算吸人氧浓度(inspired oxygen concentration,FIO2)的准确性和可行性,从而为进行氧疗的危重患者计算氧合指数(oxygenindex,OI)及病情评估提供依据和实用的方法。方法选择8名健康对照者、8例中一重度慢性阻塞性肺疾病(COPD)稳定期患者、7例ICU插管机械通气患者,吸人3种不同已知浓度的空氧混合气体,同步收集呼出气,分析FIO2与FEO2。的相关性。结果通过同步检测FEO2和FIO2,分析两者的关系,建立回归方程:健康对照组COPD组及ICU插管机械通气患者组的参数估计方程分别是:FIO2=4.02+1.02FE02—0.07RR(R^2=0.999,SE=0.659,P=0.000);F102=3.51+1.02FEO2—0.09RR(R^2=1.000,SE=0.477,P=0.000);F102=1.59+1.OOFE02(R^2=1.000,SE=0.436,P=0.000),FIO2可以很好预测F-Oz,两者相关性高,重复性好。结论通过收集呼出气测定FEO2,可准确计算出FIO2。不同试验组FIO2预测方程的决定系数非常高,标准误差小,重复性好,适合于临床评价FIO2及OI。Objective To explore the accuracies and possibilities in predicting the actual inspired oxygen concentration (FIO2) with expired oxygen concentration (FEO2), and provide evidence and practical method for calculating oxygen index (OI) and evaluating patientsr conditions during oxygen therapy. Methods Eight normal subjects,eight moderate to severe chronic obstructive pulmonary disease (COPD) patients in relatively stable condition and seven mechanical ventilated patients in intensive care unit (ICU) were recruited. The normal subjects and COPD patients were asked to breath with three different concentrations of oxygen, and the expired gas were collected simultaneously. The relationship between FIO2 and FEO2 was analyzed. Results By measuring the FEO2 and FIO2 simultaneously, relationship of two parameters was analyzed and regression equations were established. The regression equations were as following: normal subjects: FIO2 = 4.02 + 1.02FEO2 -0.07RR ( R^2 = 0. 999, SE = 0. 659, P =0. 000);COPD patients:FIO2 =3.51+1.02FEO2-0.09RR ( R2 =1. 000, SE=0. 477, P = 0. 000) ;mechanical ventilated patients in ICU:FIO2 = 1. 59+1.00FEO2 ( R2 = 1. 000, SE =0. 436, P= 0. 000). The FIO2 could be predicted with FEO2 with high correlation accuracy and repeatability. Conclusions FEO2 measued by expired gas can predict F^O2 with high accuracy and repeatability in different groups, which provides a very useful method for proper evaluating FIO2 and OI in clinical practice.
