腹腔镜与开腹子宫肌瘤剥除术的临床分析

摘　　要：目的探讨腹腔镜子宫肌瘤剥除术(laparoscopic myomectomy,LM)与开腹子宫肌瘤剥除术(transabdominal myomectomy,TAM)的临床疗效及应用价值。方法将我院2008年4月～2011年3月收治的156例子宫肌瘤患者随机分为LM组(n=82)和TAM组(n=76),对术中及术后情况等进行对比分析。结果 LM组术中出血量、术后病率、术后镇痛药物应用、肛门排气时间、术后住院时间LM组均明显低于TAM组(P<0.05),但手术时间及住院费用明显多于TAM组(P<0.05),两组术后6个月复发情况无统计学意义(P>0.05)。结论与TAM相比,LM具有创伤小、出血少、恢复快及住院时间短等优势,在一定条件下有明显的优势,但不能完全取代TAM。Objective To explore the clinical efficacy and value of laparoscopic myomectomy（LM） and tranditional myomectomy（TAM）.Methods 156 patients in our hospital from April 1,2008 to March 1,2011were randomly divided in to LM group（n=82） and TAM group（n=76）,and a series of data of were analyzed.Results The data of blood loss,postoperative morbidity,analgesia ratio,recovery time of bowel function and postoperative hospital stay in LM group is much more better than TAM group（P〈0.05）;but the operation time is much longer and hospitalization cost is much higher in LM group（P〈0.05）;there is no significant difference in relapse ratio during 6 months after the surgery（P〉0.05）.Conclusion Compared to TAM,LM has the advantage with less trauma,less bleeding,more rapid postoperative revovery and shorter hospital stay,etc,in a certain condition,but could not completely replace TAM.
