数字减影机导引下治疗椎间盘突出症双入路臭氧治疗疗效的研究

摘  要:目的:探讨在数字减影机导引下,穿刺治疗椎间盘突出症的两种入路方法及疗效的对比研究。方法:在数字减影机监视引导下,76例椎间盘突出症患者经后外侧入路行椎间盘内及盘周臭氧注射治疗;65例患者经后外侧和小关节内侧缘双入路椎间盘臭氧注射治疗。所有患者均于7d后重复治疗一次,随访6~18个月,采用MacNab腰腿痛评价标准评价其疗效情况。结果:在数字减影机导引下穿刺操作成功率为100%。按照MacNab评价标准对两种入路方式的疗效进行评价,经后外侧单入路方式治疗的有效率为75%;经后外侧及小关节内侧缘双入路治疗的有效率为84.5%。所有病例均未出现严重的并发症。结论:两种入路方式均在数字减影机导引下穿刺完成,定位准确,穿刺快捷,患者创伤小,恢复快。经后外侧和小关节内侧缘双入路椎间盘臭氧注射治疗比单纯经后外侧椎间盘臭氧注射治疗有效率更高、更稳定可靠。Objective: To investigate the effect of single and double ozone injectate in treatment of lumbosacral herniation of intervetebral disc (HIVD) under the guide of Shimadzu 10C digital subtraction angiography(DSA). Methods: 141 patients definitely diagnosed as HIVD were treated with percutaneous ozone injection under the guide of Shimadzu 10C DSA. 65 patientswere injected through the posterolateral route (single group), 76 patients were treated through the route of posterolateral and inner margin of facet joint (double group). Second injection was performed 7 days after the initial injection. All patients were followed up 6-18 months. Modified MacNab criteria was used to assessed the patients outcomes. Results: Successful punctures were obtained in all patients under the guide of Shimadzu 10 C DSA. In single group, the efficacy was 75% with the excellent in 44 cases (57.8%), good in 15 cases (17.2%) and fair/poor in 19 cases (25%). In double group, the effective rate was 84.5% with excellent in 40 cases (61.5%), good in 15 cases (23%) and fair/poor in 10 cases (15.3%). Statistical difference exhibited between these two groups. Conclusion: Both these two routes can be successful completed under the guide of DSA Percutaneous intradiscal ozone injection through the route of posterolateral and inner margin of facet joint is an effective and safe method for treatment of HIVD. It is superior to intradiscal ozone injection through the route of posterolateral alone in the treatment of HIVD.
