七氟醚低流量循环紧闭式吸入麻醉用于胸部肿瘤手术

摘　　要：目的 :报告低流量七氟醚循环紧闭式吸入麻醉用于胸部肿瘤手术 133例 ,临床观察效果满意。方法 :术中吸入低流量氧气及七氟醚麻醉 ,氧流量为 0 .5～ 1L/分 ,七氟醚最初吸入浓度为 3%～ 4% (1.8～ 2 .4MAC) ,维持吸入浓度为 1.5 %～ 2 .5 % (0 .9～ 1.4MAC)。对患者血流动力学血气分析血压和心率无创监测。结果 :七氟醚低流量吸入麻醉对患者术中血流动力学影响小 ,术中平均动脉压心率变化稳定 ,心肌耗氧量不增加。结论 :七氟醚低流量循环紧闭式吸入麻醉对循环系统影响轻微 ,用于胸部手术有良好的循环呼吸系统稳定性 ,适用于全身情况较差的肿瘤患者 ,尤其是老年肿瘤患者更为适用。Aim: To study the effect of Sevoflurane inhalation anesthesia with closed circuit and low flow on 133 cases of thoracic tumor. Methods: Low flow oxygen plus sevoflurane was given to 133 cases of thoracic tumor. The oxygen flow was 0.5～1L/m. The initial inhalated concentration of flow of sevoflurane was 3～4% (1.8～2.4MAC) and the maintenance concentration was 1.5～2.5% (0.9～1.4MAC). The hemodynamic, blood gas analysis , blood pressure and heart rate were monitored. Results: It shiowed that Sevoflurane inhalation anesthesia with closed circuit and low flow had less interference with hemodynamics of the patient, and in operation, the blood pressure and heart rate were stable and the cardiac oxygen consumption did not increase. Conclusion: Sevoflurane inhalation anesthesia with closed circuit and low flow has a good effect to stabilize the respiro-circulatory function of thoracic tumor patients during operation, especially the poor condition elderly patients.
