摘 要:目的 研究失血性休克时全身氧供给 (DO2 )与氧消耗 (VO2 )关系的变化。方法 按 4ml/kg、每隔 0 5小时分次从静脉放血 ,观察大鼠休克过程中DO2 、VO2 、氧摄取率(ExtO2 )等变化。结果 失血 2 0小时平均动脉压开始明显下降 (P <0 0 1)。在休克早期 ,DO2随进行性下降 ,ExtO2 出现代偿升高、VO2 维持相对不变 ,呈非氧供依赖关系 ;当DO2 降至2 7 6 4ml·min- 1 ·kg- 1 后、VO2 随DO2 线性降低 (r=0 788,P <0 0 1) ,即呈氧供依赖关系 ,并伴血乳酸浓度升高。结论 失血性休克时DO2 与VO2 间呈双相变化关系 。Objective To explore changes of the relationship between systemic oxygen delivery(DO 2) and oxygen consumption(VO 2) during hemorrhagic shock.Methods The model was established by withdrawing venous blood by 4ml/kg at half hourly interval.DO 2、VO 2 and oxygen extraction(ExtO 2) were measured in hemorrhagic shock rats.Results Mean arterial pressure was lowed significantly(P<0 01) at 2 0 hour of blood letting. During the early period of hemorrhagic shock,DO 2 decrcased progressively,associated with compensatory increase in ExtO 2.VO 2 remained relatively stable which was independently to DO 2.Linear reductions in VO 2 with DO 2(r=0 788,P<0 01) existed when DO 2 was below 27 64ml·min -1 ·kg -1 .It was called dependence of VO 2 on DO 2 associating with elevated blood lactic acid concentration.Conclusion Biphasic relationship between DO 2 and VO 2 exhibited.The existence of dependence of VO 2 on DO 2 is an indicative of body tissue hypoxia.
