摘 要:目的研究实验性矽肺大鼠在染尘后不同时间肺泡灌洗液(BALF,bronchoalveolar lavage fluid)中巨噬细胞和白介素-6(IL-6)的变化,并探讨其意义。方法将54只健康雄性SD大鼠随机分为模型组27只、对照组27只。模型组以非暴露方式一次性气管内注入1 ml二氧化硅悬液(40 g.L-1)建立大鼠矽肺模型,对照组气管内注入等量的灭菌生理盐水。分别在染尘后第1、7、14、21、28、35、77、106、126天处死各组大鼠各3只,收集肺泡灌洗液和肺组织,用免疫组化S-P法观察肺组织中NF-κB的表达情况;ELISA法检测BALF中IL-6的含量;并计数有核细胞总数和巨噬细胞百分比。结果模型组大鼠BALF中IL-6的浓度在早期(第1、7、14天)均比对照组高,第7天下降,第14天最高,差异有统计学意义(P<0.05),其后下降到低位,后期有所回升,差异无统计学意义。染尘大鼠有核细胞计数和巨噬细胞百分比计数均比对照组大鼠的高,差异均有统计学意义(P<0.05)。有核细胞计数在第14天增高并在后期(第77、106、126天)直线上升;巨噬细胞百分比计数在第14天和第126天达到峰值。肺组织中NF-κB的表达在第1、7、14天与第77、106、126天时明显增加,有统计学意义(P<0.01),第21、28、35天时,没有显著差异,模型组均高于对照组。结论肺泡巨噬细胞在接触硅尘后变化和分泌炎症因子与矽肺的发生发展有密切关系。Objectlve To investigate the changes and significance of macrophages and IL-6 in BALF at different stages of rat silicosis models. Methods Fifty-four male SD rats were randomly divided into the silica exposed group and the control group. One ml silica suspension (40 g . L-1 ) was administer via intratracheal instillation once while equal amount of saline was installed to the rats of control group. Animals were sacrificed at the 18t ,Th, 14th ,21 st ,28th ,35th ,77th 106th 126th day after silica instillation, the samples of BALF and lung tissues were collected. The expression of NF-KB was observed by use of immunohistochemistry method;the contents of IL-6 in BALF were assayed by ELISA. The nucleated cell number was counted and the percentage of macrophage was calculated. Results The levels of IL-6 in BALF of silicotic rats was higher than those in control group in early stage ( 1st ,7th, 14th,day). IL-6 declined on the 7th day, peaked on the 14th day, there was statistics difference ( P 〈 0.05 ), then IL-6 deceased afterwards. During the late stage of experiment ( 77th, 106th ,126th day) it increased and the difference was not obvious. The nucleated cell count and the percentage of macrophage of silicotic rats in BLAF were higher than those of control group at various periods and the difference was obvious ( P 〈 0.05 ). The nucleated cell count rose on the 14th day and straightly increased in the late stage of experiment (77th, 106th, 126th day). There emerged two peaks of the percentage of macrophage respectively at 14'h day and 126th day. In the early stage of experiment (1st ,7th, 14th day) and late stage of experiment (77th, 106th, 12Ch day), the levels of NF-KB in lungs of silicotic rats were obviously higher than those in control group (P 〈 0.01 ). In other stage of experiment (21st ,28th ,35th day), there was no obvious difference but the levels were still higher than those of control group. Conclusion The change of alveolar macrophages exposed to silica dust
