摘 要:目的观察ATP-红外生物效应技术对难治性慢性外阴瘙痒的治疗效果及其安全性。方法采用前瞻性对照研究方法,共有80例符合入选标准的病例进入研究。将患者分为两组:A组(外阴鳞状上皮增生)54例,B组(外阴硬化性苔藓)26例。采用ATP-红外生物效应治疗仪照射外阴部,开始每天一次,待症状缓解后,治疗间隔逐渐延长,至症状完全消失后停止治疗。记录治疗后外阴瘙痒症状的变化,通过阴道镜观察外阴皮损的变化情况。结果治疗前瘙痒总积分与瘙痒显效时间有关,治疗前瘙痒总积分与皮损显效时间有关。A组和B组瘙痒显效及皮损好转的中位时间差异无统计学意义(P>0.05)。所有患者在治疗过程中均未发现不良反应。结论 ATP-红外生物效应技术对难治性慢性外阴瘙痒患者的瘙痒症状及皮损恢复均有良好的治疗效果,且安全可靠。Objective To observe the clinical efficacy of ATP-infrared biological treatment in patients with refractory chronic vulvar pruritus. Methods A prospective and controlled study was performed in 80 patients with chronic vulvar pruritus, including 54 cases of squamous hyperplasia (group A) and 26 cases of linchen sclerosis (group B). ATP-infrared biological treatment was adopted daily at first. The treatment interval was gradually lengthened with the relieving of symptoms. The treatment lasted until the patients were fully recovered. Changes of vulvar itching were recorded daily. Colposcolpy was performed in every patient to observe the lesions. Cox regression was applied to analyze the relevant factors influencing vulvar itching and skin scratch. Results Before treatment, the total scores of pruritus were positively related to the effective time for itching (P=0.006). The bigger the sum scores of pruritus, the longer the effective time for itching. Before treatment, the total scores of pruritus were positively related to the effective time of lesions (P =0.001). No statistically significant difference was found between the two groups for both vulval itching and lesions (P〉0.05). Among the 67 cases who achieved effects, 58 of them were followed up for 4~18 months. Vulvar itching was found relapsed in 13 cases within 3~12 months. Nine of them repeated the treatment and all received good results. No adverse effects were found during the treatment. Conclusion ATP-infrared biological treatment is effective in controlling vulvar itching and lesions in patients with refractory chronic vulvar pruritus.
