摘  要:目的探讨双蒂腓肠肌皮瓣在修复跟腱及皮肤缺损中的临床应用。方法选取2007年4月~2010年12月在某院治疗的30例小腿跟腱及皮肤缺损患者为研究对象,随机将患者分为实验组(双蒂腓肠肌皮瓣组)16例和对照组(传统手术方法组)14例,比较两种手术方法的临床疗效,并行统计学分析。结果实验组术后患者平均手术时间为(32.3±4.5)min、术后感染率为6.25%,明显低于对照组;伤口一期愈合率及膝和髁关节正常活动率均为100.00%,均明显高于对照组;二次手术及出院后再次出现跟腱断裂的患者比例均为0.00%,明显低于对照组。以上P值均小于0.05,差异有统计学意义。结论双蒂腓肠肌皮瓣在修复跟腱及皮肤缺损中的临床应用效果良好,手术方法简单,不需要二次手术,安全性较高,可以推广应用。OBJECTIVE To investigate the clinical application of double pedicle gastrocnemius flap in the repair of achilles tendon and skin defects.METHODS 30 cases treated with calf achilles tendon and skin defects in our hospital from April 2007 to December 2010 were chosen as the research objects.They were randomly divided into the experimental group(double pedicled gastrocnemius flap group)of 16 cases and the control group of 14 cases(traditional surgery group).The clinical efficacies of the two surgical method were compared and taken for statistical analysis.RESULTS The mean operative time of the experimental group patients was 32.3 ± 4.5 minutes,and the postoperative infection rate was 6.25%,it was significantly lower than the control group.The wound healing rate of a knee and normal activity rate of ankle joints were 100.00%,and were significantly higher than the control group.The second surgery and discharge from the proportion of patients with achilles tendon rupture was 0.00%,was significantly lower than the control group.P﹤0.05,there was a significant difference.CONCLUSION The clinical application of double pedicle gastrocnemius flap in the repair of achilles tendon and skin defects is good,and the surgical method is simple,needn't a secondary surgery.It is with high security,can be used in the clinical application.
