单孔电视胸腔镜在自发性气胸中的临床应用

摘　　要：目的探讨单孔电视胸腔镜手术(video assisted thoracoscopic surgery,VATS)治疗自发性气胸的临床价值。方法 2008年4月至2011年4月,对107例自发性气胸行单孔VATS下肺大疱切除及胸膜固定术。结果 107例手术均成功,无中转开胸,切开辅助孔9例。手术时间20～100min,平均36min;术中出血<50ml。术后引流时间2～4d,平均3d;术后住院4～6d,平均5d。103例随访2～36个月,平均27个月,1例复发。结论单孔VATS安全可靠、创伤更小。Objective To discuss the application of video assisted thoracoscopic surgery through single foramen for spontaneous pneumothorax.Methods Video assisted thoracoscopic surgery through single foramen and pleurodesis were ocurred in 107 patients who suffered from spontaneous pneumothorax.Results The operations are all successful,and no thoracotomy ocurred halfway,and incision of via hole ocurred in 9 patients.Operation time was 20~100min,mean time was 36 min.Bleed in operation was under 50 ml.Drainage time after operation was 2~4 days,mean time was 3 days.Hospitalized time was 4~6 days,mean time was 5 days.103 patients were followed-up for 2~36 months,mean time was 27 months.And 1 patient recurred.Cnclusions Video assisted thoracoscopic surgery through single foramen is safe and reliable,and trauma is little.
