下腔静脉滤器在下肢深静脉血栓形成导管溶栓术联合中医药治疗中的应用

摘　　要：目的:评价下腔静脉滤器对急性下肢深静脉血栓形成介入联合中医综合治疗中的作用,探讨其临床应用价值。方法:回顾性研究2000—2011年在广州中医药大学第一附属医院诊治的60例急性下肢深静脉血栓形成(lower extremity deep venous thrombosis,LEDVT)病例。所有患者均行介入综合治疗。随访1年以上,按照疗效标准观察各项指标。结果:45例置入下腔静脉滤器,成功率100%,随访期内均未发生移位、严重倾斜、展开不良等严重并发症。两组术后1个月、6个月临床疗效比较差异均有统计学意义(P<0.05);血管通畅度比较差异有统计学意义(P<0.05)。结论:下腔静脉滤器可预防致命性肺动脉栓塞(pulmonary embolism,PE)的发生并提高LEDVT介入联合中医综合治疗中的溶栓药物安全性,间接提高溶栓效果及临床疗效。Objective ： To evaluate the clinical effectiveness of inferior vena cava filter in percutaneous catheter thrombus aspiration combined with traditional Chinese medicine （TCM）to treat lower extremity deep venous thrombosis. Methods：From 2000 to 2011,60 cases with acute LEDVT determined by contrast venography in the First Affiliated Hospital of Guangzhou University of Chinese Medicine were studied retrospectively, treated with integration of interventional radiology and syndrome differentiation of TCM. Duration of follow-up was beyond one year. Results ： The filters and the stents were inserted successfully into 45 patients, respectively. There were n＇o clinical manifestations of pulmonary embolism or other complications during follow-up. There were significant differences in statistics on the clinical effectiveness and venous lumen size compared with no filter group during 1st month and 6th month follow-up. Conclusion：The application of inferior vena cava filter in percutaneous catheter thrombus aspiration combined with traditional Chinese medicine to treat lower extremity deep venous thrombosis is safe and can increase efficacy of thrombolysis and prevent pulmonary embolism.
