MRI对脊髓结核球的诊断价值

摘　　要：目的探讨脊髓结核球MRI表现,提高对该病的影像认识。方法回顾性分析20例脊髓结核球的MRI影像资料,观察指标为病灶的数量、分布、大小、形态、信号特点及伴随征象。结果 20例患者共检出结核球病灶47个,位于颈段脊髓13个、胸段脊髓内16个,腰段脊髓及马尾神经区18个,好发部位为颈膨大、腰膨大区(P值分别为0.001和0.008);病灶大小0.2～3.5cm,平均最大径1.1cm;病灶分圆形、类圆形、不规则形。脊髓结核球MRI表现分未成熟结核结节和成熟结核结节两种类型,未成熟结节32个,成熟结节15个,未成熟结节全结节呈较均匀一致强化,成熟结核结节病灶中心出现坏死,增强扫描为环状强化。结论 MRI平扫和增强扫描可准确、清晰地显示脊髓结核球的形态特征,可为临床治疗及判断预后提供可靠的影像学参考。Objective To investigate MRI diagnosis of tuberculoma in spinal cord. Methods Radiological data of plain and enhanced MRI in 20 spinal cord tuberculoma cases were reviewed retrospectively, including the amount, disposition, size, shape, signal characteristic and accompanying signs. Results In 20 cases, 47 lesions were detected, 15 of which were localized in cervical spinal cord, 14 of which were localized in thoracic spinal cord, the rest 18 were localized in lumbar spinal cord and cauda equine. Most lesions were detected in the enlarged cervical and lumbar spinal cord（P values were 0. 001 and 0. 008 respectively）. Lesions size varied from 0. 2 to 3.5 cm,the mean diameter was 1.1 cm. The shapes of lesions were round, oval and irregular. There were two kinds of tuberculomas presented by MRI, immature tuberculomas （32 of 47 ）and mature tuberculomas（ 15 of 47）, uniform increased signal of immature tuberculomas were seen on the enhanced MRI. While increased rim of mature tuberculomas were seen because of the necrosis inside the lesions. Conclusions The morphological characteristic of tuberculoma in spinal cord could be shown clearly by plain and enhanced MRI. MRI played an important role in tuberculoma＇s therapy and prognosis.
