健康儿童全血细胞分析11项参数年龄差异调查

摘　　要：目的探讨实验室儿童血细胞11项分析参数年龄组间的差异对参考范围设置的影响。方法收集1274例1～12岁健康查体儿童静脉血，采用自动血细胞分析仪进行检测，获得血细胞分析白细胞、红细胞、血红蛋白、红细胞比积、血小板数、红细胞平均体积、平均血红蛋白含量、平均血红蛋白浓度、中性粒细胞比率计数、淋巴细胞比率计数、单核细胞比率计数11项参数，并将其分为1～岁、3一岁、6—12岁3个年龄组进行统计学分析。结果1～岁组与3～岁组比较，白细胞、红细胞比积、平均血红蛋白含量、红细胞平均体积、中性粒细胞比率计数、淋巴细胞比率计数的均值存在显著性差异（t值分别为2．528、2．490、2．128、3．442、8．072、6．939，均P〈0．05）。3一岁组与6—12岁组比较，白细胞、红细胞、血红蛋白、红细胞比积、红细胞平均体积、平均血红蛋白浓度、中性粒细胞比率计数、淋巴细胞比率计数的均值存在显著性差异（t值分别为3．291、3．259、8．323、9．327、7．070、6．784、10．890、11．189，均P〈0．01），血小板数的均值存在显著性差异（t=1．989，P〈0．05）。结论健康儿童全血细胞11项分析参数比较结果提示有8个分析参数存在年龄组差异，建议白细胞、中性粒细胞比率计数、淋巴细胞比率计数参数可分3个年龄段设置参考范围，红细胞、血红蛋白、红细胞比积、红细胞平均体积、平均血红蛋白含量参数可根据各自实验室分析数据设置参考范围。Objective To investigate the influence of 11 hematological parameters of healthy children among different age groups on reference range. Methods Automated hematological analyzer was used to detect the venous blood of 1 274 healthy children aged 1-12, and 11 parameters including WBC, RBC, HGB, HCT, PLT, MCV, MCH, MCHC, NEUT%, LYMPH% and MONO% were selected. The data was analyzed within 3 age groups, group of 1-year old, group of 3-year old and group of 6-12 years. Results There were significant differences in WBC, HCT, MCH, MCV, NEUT% and LYMPH% between group of 1-year old and group of 3-year old （ t value was 2. 528, 2. 490, 2. 128, 3. 442, 8. 072 and 6.939, respectively,all P 〈 0.05）. WBC, RBC, HGB, MCV, MCH, MCHC, NEUT% and LYMPH% of group of 3-year old were significantly different from those of group of 6-12 years （ t value was 3. 291,3. 259, 8. 323, 9. 327, 7. 070, 6. 784, 10.890 and 11. 189, respectively,all P 〈 0.01 ） , and the mean of PLT between two groups was also different significantly （ t = 1. 989,P 〈 0.05 ）. Conclusion The results of hematological analysis of 11 parameters indicate that there are differences in 8 ones among different age groups. Therefore, it is suggested to set reference range for WBC, NEUT% and LYMPH% according to three age groups, and the reference range for RBC, HGB, HCT, MCV and MCH can be set in accordance with experimental data.
