体外受精-胚胎移植术后宫角妊娠38例临床分析

摘  要:目的探讨体外受精-胚胎移植后发生宫角妊娠的高危因素及诊断和治疗措施。方法收集38例体外受精-胚胎移植后发生宫角妊娠患者,与同期体外受精-胚胎移植获得正常妊娠患者相比较,分析宫角妊娠主要发病因素及临床诊治效果。结果38例宫角妊娠患者中,输卵管因素29例,占76.32%,其中输卵管切除14例,占36.84%。宫角妊娠组新鲜周期取卵数(22.1±7.5枚)明显多于正常妊娠者取卵数(13.2±6.2枚),差异具有显著性(t=0.285,P〈0.05);宫角妊娠组移植日雌激素值(2829±951pg/mL)明显高于正常妊娠组(2320±846pg/mL),两组数据相比均有统计学差异(t=1.633,P〈0.05)。结论输卵管因素尤其是输卵管切除是体外受精术后宫角妊娠的高危因素,较高的取卵数及移植日高雌激素水平可能是导致宫角妊娠发生的原因之一。Objective To explore the high-risk factors, diagnosis and treatment of cornual pregnancy after in vitro fertilization and embryo transfer (IVF-ET). Methods The data of thirty-eight patients with cornual pregnancy after IVF-ET was collected to analyze the major pathogenic factors and the treatment effect by comparing with those with normal pregnancy after IVF-ET. Results Of 38 cases with cornual pregnancy, 29 (76.32%) were caused by tubal factors, including 14 cases (36.84%) of salpingeetomy. In carnual pregnancy group, the retrieval number of oocyte in fresh cycle (22.1 ± 7.5 ) was obviously more than that (13.2 ± 6.2) in normal pregnancy group, and the difference was significant (r= 0.285,P 〈 0.05 ). The estrogen level at transplanting day in cornual pregnancy group (2 829 ± 951 pg/mL) was significantly higher than that (2 320 ± 846 pg/mL) in normal pregnancy group, and there was statistical difference ( t = 1. 633 ,P 〈 0.05 ). Conclusion Tubal factors especially salpingectomy are the high-risk factors of corneal pregnancy after IVF, and high retrieval number of oocyte and estrogen level at transplanting day are one of reasons causing corneal pregnancy.
