单侧声带麻痹患者的嗓音主观RBH评估与DSI的相关性研究

摘　　要：目的:探讨嗓音主观RBH评估与发音障碍严重指数(dysphonia severity index,DSI)用于单侧声带麻痹患者的临床意义及其相关性,并进一步验证DSI的实用性。方法:对45例单侧声带麻痹患者(患者组)及13例嗓音正常人(对照组)进行主观RBH评估和DSI测定。主观RBH评估分四个等级;DSI测定由公式DSI=0.13×MPT+0.0053×F(0)-High-0.26×I-Low-1.18×Jitter(%)+12.4计算得来,也分四个等级。比较单侧声带麻痹患者RBH分级和DSI分级间差异。结果:患者组的R、B、H均高于对照组,差异有统计学意义(P<0.05);患者组DSI的平均值0.35±0.74,而正常组的平均值为4.23±0.81,两组比较差异有统计学意义(P<0.05)。单侧声带麻痹患者主观RBH评估与DSI分级之间相关性显著。结论:选择DSI作为嗓音障碍评估的客观参数,应用于单侧声带麻痹患者具有较好的临床应用价值,其与主观评估参数R、B、H具有很好的相关性。Objective：To explore the correlation between dysphonia severity index（DSI） and voice perceptual evaluation RBH system in patients with unilateral vocal cord paralysis,and validate the usefulness of the DSI.Methods：The voice samples were collected from 45 patients（patient group） with unilateral vocal paralysis and 13 subjects with normal voice（control group）.They all were tested by RBH assessment and DSI determination.RBH evaluation is graded into four scales from 0 to 3;DSI=0.13×MPT＋0.0053×F（0）-High-0.26×I-Low-1.18×jitter（%）＋12.4 calculated to,it is graded into four scales also.SPSS 11.5 statistical software was adopted to analysis all of the datum in the study to show whether there are significant correlation between the RBH and DSI classification.Results：R,B,H in the patient group were higher than the control group,the difference was obvious（P0.05）;DSI in the patient group was average of 0.35±0.74,while the control group was 4.23±0.81,the difference between the two groups was statistically significant（P0.05）.The correlation between RBH assessment and DSI was significant in patients with unilateral vocal cord paralysis（P0.01）.Conclusion：It is feasible to use DSI to measure the severity of the dysphonia in patients with unilateral vocal cord paralysis,and has good correlation with the perceptual evaluation parameters R,B and H.
