摘 要:目的观察正常听力耳鸣患者的耳鸣音调、响度、掩蔽曲线及高刺激率听觉脑干反应(ABR)检测结果,探讨耳鸣的临床特征及发病机制。方法选取162例正常听力耳鸣患者,对其进行耳鸣音调、响度匹配,同时选取其中140例单侧耳鸣患者,对140患耳与对侧140健耳,分别进行高、低刺激率ABR检测,比较不同刺激率下PL差值(△PL)、IPL差值(△IPL)。结果听力正常耳鸣的耳鸣音调分布从125~8 000 Hz,以4 000~8 000 Hz高频为主;耳鸣响度从0~20 dB,5~10 dB最多;佛德曼曲线以汇聚型为主;患耳与健侧耳高、低刺激率ABRⅠ~Ⅴ△IPL差值>0.28 ms的耳数分别为16耳及12耳,差值无显著意义(P>0.05)。结论总结162例听力正常耳鸣患者的临床特征,为进一步的临床诊疗提供依据;耳鸣起源于内耳微循环障碍的理论不适用于解释正常听力耳鸣患者,耳鸣产生的机制可能不能用单一的理论解释。Objective To investigate the clinical characteristics of tinnitus and the relationship between inner ear micro-circulation and tinnitus by examining tinnitus pitch-matching, loudnessin-matching, Feldmann masking curves and high stimulus rat ABR in tinnitus patients with normal hearing. Methods 162 patients with normal hearing and 140 tinnitus patients were enrolled. Tinnitus measurements consisted of pitch-matching, loudnessin-matching, Feld- mann masking curves, and high stimulus rat auditory brainstem response (ABR). Results The tinnitus pitch- matched frequencies of tinnitus patients with normal heating ranged from 125 Hz to 8 000 Hz,in which 57.3% of them located between 3 000 Hz - 8 000 Hz. The tinnitus loudness-matched frequencies ranged form 0 dB to 2 0 dB, in which 45.6% of them were 5 - 10 dB. There was no difference in I - V interval between the tinnitus ears and normal ears. Conclusion The clinical characterisitics of 162 tinnitus patients with normal hearing were identified in or- der to facilitate the further management. The relationship between the inner ear micro-circulation and tinnitus could not be attributed to the tinnitus in patients with normal hearing.
