摘 要:目的:评价不同方式联合窄波UVB照射对于自体黑素细胞培养移植治疗白癜风疗效的影响。方法:将符合纳入标准的稳定期白癜风患者随机分为四组,A组:移植前进行窄波UVB照射20次;B组:移植后予窄波UVB照射30次;C组:移植前予窄波UVB照射20次,移植后再予窄波UVB照射30次;D组:仅进行自体黑素细胞移植,未照射窄波UVB。治疗后至少随访6个月观察复色效果。结果:共治疗221例白癜风患者,移植前后均照光组48例,有81.3%的患者获得90%-100%的复色,获得50%以上复色率的患者达93.8%;移植前照光组54例患者的痊愈率为68.5%,有效率为92.6%;移植后照光组63例患者的痊愈率为61.9%,有效率为88.9%;仅移植未照光组56例,仅有44.6%的患者获得90~100%的复色,获得50%v7,上复色率的患者为73.2%。四组的痊愈率和有效率均有统计学差异。结论:体外培养的自体黑素细胞移植治疗稳定期白癜风疗效肯定,用少量供皮区即可治疗大面积皮损,联合窄波UVB治疗可显著提高疗效,其中移植前后均进行窄波UVB照射者效果最佳。Objective To evaluate the therapeutic effect of transplantation of autologous melanocytes transplantation combined with different modalities of NB-UVB in treating vitiligo. Methods Patients with stable vitiligo were randomly assigned to different study groups according to a continuous selection method over a period of three years.Group A was given 20 times of NB-UVB before transplantation;Group B was given 30 times of NB-UVB after transplantation.Group C was given 20 times of NB-UVB before transplantation as well as 30 times of NB-UVB after transplantation.Group D only received transplantation,All patients were followed up for at least 6 months. Results Group C responded best,more than 90% repigmentation was achived in 81.3% patients, and 93.8% patients experienced 50% or more repigmentation in this group.The cure rate was 68.5% and 61.9% in Group A and Group B,respectively.The effective rate was 92.6% and 88.9% in Group A and Group B,respectively. Group D showed the poorest results,with the cure rate of 44.6% and the effective rate of 73.2%.A homogeneous skin color was obtained and no scarring or other serious side effects were observed. Conclusion Transplantation of cultured autologous melanocytes is an effective modality for the treatment of stable vitiligo,with the advantage of treating large depigmented area with melanocytes from a small donor site. And in association with NB-UVB can improve the transplantation's effectiveness,especially given NB-UVB both before transplantation and after transplantation.It is valuable in clinical application.
