摘  要:目的研究血浆脑钠肽(BNP)在急性ST段抬高性心肌梗死(AMI)患者的应用价值及其与梗死范围和心肌梗死后相关合并症的关系。方法在本研究里,使用酶联免疫法测定35例急性ST段抬高性心肌梗死患者和20例正常对照血浆BNP水平,观察BNP与AMI患者梗死范围、心功能、心律失常合并症及左室射血分数(LVEF)的关系。结果 AMI患者BNP水平明显高于对照组(P<0.01);前壁+/前间壁及侧壁梗死患者BNP最高(222.66±200.60pg/ml);AMI患者心功能越差,BNP水平越高,BNP与LVEF呈负相关(r=-0.828,P=0.000);合并室速的AMI患者BNP水平最高,并具有统计学差异(P=0.003)。结论AMI发生后,血浆BNP水平是左室功能、心律失常合并症及LVEF一个重要的预测指标。Objective To investigate the prognostic value of B - type natriuretic peptide (BNP) in acute myocardial infarction (AMI) patients and study its relation with left ventricular function and post - myocardial infarction complications. Methods In this stud- y, plasma BNP level was measured by the way of ELISA in 35 patients with acute ST elevation myocardial infarction (AMI) and 20 normal adults, age and gender matched subjects. The relationship between serum BNP level and cardiac function,infarction areas,electrical compli- cations and left ventricular ejection fraction were obtained in patients with AMI. Results BNP level in AMI patients were significantly high- er than control group( P 〈0. 01 ). The highest BNP level was in anterior- lateral wall AMI(222.66 + 200.60pg/mL). The serum BNP level was significantly higher along with the cardiac function worse. There was significant reverse relation between BNP and LVEF ( r = - 0. 828 ,P = 0. 000). The highest BNP level was detected in ventricular tachycardia (P =0. 003). Conclusion After the onset of AMI, plasma BNP can be used as an important predictor for left ventricular dysfunction, arhythmia complications and left ventricular ejection fraction.
