丹皮酚软膏对老年性皮肤瘙痒症免疫偏移的影响

摘　　要：目的探讨丹皮酚软膏对老年性皮肤瘙痒症患者Th1、Th2免疫偏移的影响。方法收集2009年1月至2011年12月于该院就诊的泛发型老年性皮肤瘙痒症患者80例,40例为基础治疗组,仅口服氯雷他定;40例为试验组,在口服氯雷他定的基础上,外用丹皮酚软膏。同期收集40例健康老年人作为对照组。采集各组患者治疗前后及健康志愿者的血液标本,ELISA法检测细胞因子IFN-γ、IL-2、TNF-α、IL-4、IL-5、IL-10的表达水平。结果老年性皮肤瘙痒症患者IFN-γ、IL-2、TNF-α等Th1细胞因子的表达均高于健康对照组(P<0.05);而IL-4、IL-5、IL-10等Th2细胞因子的表达均低于健康对照组(P<0.05);治疗后与治疗前比较,口服氯雷他定组患者IFN-γ、IL-2、TNF-α等Th1细胞因子无明显变化(P>0.05),IL-4、IL-5、IL-10等Th2细胞因子的表达高于治疗前(P<0.05);外用丹皮酚组患者IFN-γ、IL-2、TNF-α等Th1细胞因子低于治疗前(P<0.05),IL-4、IL-5、IL-10等Th2细胞因子的表达高于治疗前(P<0.05)。结论老年性皮肤瘙痒症患者存在不同程度的Th1/Th2免疫偏移,表现为Th1型细胞因子功能亢进,Th2型细胞因子功能降低;丹皮酚能纠正此免疫偏移状态。Objective To investigate the effect of paeonol ointment on Thl, Th2 immune deviation of elderly patients with pruritus. Methods Eighty cases of elderly patients with pruritus from January 2009 to December 2011 were collected, and were randomly divided into two groups. Forty cases were the basement treatment group, only were given oral loratadine. Another 40 cases were the experimental group, which were given paeonol ointment on the basis of oral loratadine. At the same period, 40 cases of healthy elderly patients were collected as the control group. Blood specimens were collected before and after the acquisition of each group and healthy volunteers. The levels of IFN-γ, IL-2, TNF-α, IL-4, IL-5, IL-10 were measured by ELISA. Results The levels of IFN-γ, IL-2 and TNF-alpha in senile pruritus patients were higher than those in healthy control group （ P 〈 0. 05 ）, but the levels of IL-4, IL-5, IL-10 were lower than those in healthy control group （ P 〈 0. 05 ）. The levels of IFN-γ, IL-2 and TNF-alpha in oral loratadine group after treatment had no significant changes compared with those of before treatment （ P 〉 0. 05 ）, but the levels of IL-4, IL-5, IL-10 were higher than before （ P 〈 0. 05 ）. The levels of IFN-γ, IL-2 and TNF-alpha in paeonol ointment group were lower after treatment than before （P 〈 0.05 ）, but the levels of IL-4, IL-5, IL-10 were higher after treatment than before （ P 〈 0. 05 ）. Conclusions There exist varying degrees of Thl/Th2 immune deviation, including Thl -type eytokines hyperthyroidism and Th2-type eytokines reduction in senile pruritus patients. The paeonol could correct the immune deviation of senile pruritus patients.
