摘 要:目的:研究参芪保肾颗粒剂对慢性肾衰竭(CRF)氮质血症期的患者肾功能、肾纤维化的作用。方法:对60例原发病为慢性肾小球肾炎发展致CRF氮质血症期的患者进行了随机对照研究,从临床角度观测参芪保肾颗粒剂对CRF氮质血症期患者的肾小管损伤检测指标NAG(N-乙酰-β-D-氨基葡萄糖甘酶)、Trf(转铁蛋白)、β2-MG(β2-微球蛋白)及Upr24(24小时尿蛋白定量)、Hb(血色素)、Scr(肌酐)、BUN(尿素氮)等指标的影响。结果:参芪保肾颗粒剂能下调NAG、β2-MG、Trf、Upr24、Scr、BUN指数,通过降低减缓肾小管-间质纤维化及瘢痕组织增生,起到保护肾小管-间质的功能的作用,有效保护肾小管的损伤,起到延缓CRF进展的作用,治疗前后有显著性差异(P<0.05)。结论:脾肾两虚为CRF之主要病机,益气健脾补肾为主应为治疗CRF及延缓其进展的主要方法。慢性肾小球肾炎的肾功能减退更密切相关于间质的纤维化而非肾小球损伤,从保护肾小球间质方面入手是延缓CRF进展的更有远期预防和治疗意义的重要治疗手段。参芪保肾颗粒剂对缓解贫血状态,减少蛋白的流失,保护肾小管的功能,防止间质纤维化有较好的疗效,起到延缓CRF进展保护肾功能的作用。Objective : To study Shenqi Baoshen Granules' effect on kidney function and kidney fibrosis of patients suffer- ing CRF. Method: A random comparative research was conducted on 60 patients suffering CRF which developed from slow glomerulone nephritis and Shenqi Baoshen Granules' s clinic effect on renal tubules injury check indexes including NAG, Trf, B2 - MG, Upr24, Hb, Ser and BUN was observed on sufferers of CRF. Results: Shenshi Baoshen Granules could lower NAG, Trf, B2 - MG, Trf, Upr24, Ser and BUN and effectively protected the function of renal tubules and renal tubules injury by reducing and delaying renal tubules fibrosis and hyperplasia of scar organization, thus delayed the development of CRF. There were significant differences before and after the treatment (P 〈 0. 05 ). Conclusion : Weak- ness of spleen and kidney is the main cause of CRF. Strengthening Qi, protecting spleen and supplementing kidney can be the main method to cure CRF and delay its development. Deterioration of Kidney function of slow glomerulus nephritis is more due to renal interstitial fibrosis than glomerulone injury. To protect renal tubules is a far more long -term pre- vention and treatment approach to delay the development of CRF. Shenshi Baoshen Granules have a good effect on alle- viating anemic, reducing the losing of protein, protecting the function of renal tubules and preventing renal tubules fibrosis while delaying the development of CRF and protecting the kidney function.
